Yesterday, I upgraded my courtyard to an infirmary except...everything looked the same. The only difference is that the surgeon character who was down by the tents is now missing. It's entirely possible that I'm overlooking something obvious, but I can't find any area that shows an upgrade.

I also changed the tower and nothing changed then i went to the building behind Cassandra and he didnt say anything

There are some steps next to the infirmary area tht go into a tower like thing, next to the gate. Go into that and I believe that is what was upgraded. I was the same as you till I looked around....

You do the Requisitions until you come to a Skyhold Upgrade. Then you have to craft it and go from there, depending on what it is, it will ask you what you want to upgrade: infirmary or (cant remember, i think barracks?) and then it does it.
