I used one of my inquisitor perks to gain an extra potion bottle and found a belt that granted me two extra non healing potions and yet I still only get the default number of potions.

 

What do I have to do to gain the extra potion slots?

Not certain, but the belt adds another potion to the slot, while the Perk may open up an extra slot (ie; 3rd). However, the belts are glitched a little; seem to lose a flask if using Fast Travel, or after Rest.
