I've been bouncing back and forth with this for a while now.  I play on Nightmare, as a mage.  I mostly have Fire and Lightning elements, with a bit of Barrier.  But I also bring Solar, who also has a Barrier, and a full slate of Cold magic.  Sometimes I roll with Cassandra/Blackwall and then Varrick or Sera for their DPS and hard hitting one-shots.  However, I've also had really good luck putting Bull in as well (he's built to have lots of guard).  Having Cassandra or Blackwall, along with Bull, and the two mages has worked out really well in some situations, mostly because of the aggro they remove from the mages.

At the same time, Varrick and Sera have some incredible damage on certain attacks, including the ability to put some tough enemies asleep for a short time.

What's everyone having good luck with, particularly on the Nightmare setting when you need to micromanage every step of combat?

I tried a 2H and S&S together, but the 2H lost Health far too quickly unless micro-managed. Of course, I am playing a Rogue, so this might result in a better outcome if one is controlled by the Player.

As a Sword and Board Quizzy I had Blackwall or Iron Bull in my party for a while, but it was too much work. I switched to Solas and Dorian (plus a rogue), it works much better.

I run cassandra and blackwell ( as a 2he ) and usually either varic or a dual wield rouge. I play a mage. Seems to work alright as long as I keep a shield up. For really challenging parts I just replace the rouge with another mage to make sure shields are never down.

Playing on Hard and rolling with 2x shields and 1x mage as company for my dagger rogue at the moment (level 7ish). The only thing I really miss is being able to kick off a fight with upgraded Full Draw.

On nightmare, My main character is a mage.
I send Blackwall to face tank the world (which he does an amazing job of).

I have Sera and Solas around, both set to not drink my potions. (Those are for me and maybe Blackwall goddamnit >_<)

They damage till they die. Blackwall never dies, and my mage usually drinks most of my potions.

My party consistently has Cassandra (Sword + Shield) and Iron Bull (2H).  Iron Bull acts a damage sponge and keeps enemies down, while Cassandra taunts and boosts party morale.  I will toss in ice spec Solas or spirit spec Dorian for the most part.. and then sometimes another party member when I just need variety.  Cold and Barrier mage + warriors works very well for keeping enemies away and me not having to worry about casting Barrier on my tanks, as I have the skill that lets you use barrier for more damage.  Spirit spec with revive has been a lifesaver once in awhile.

I sometimes throw in Blackwall (Sword + Shield) as well when I *really* need damage soakers and attention drawn elsewhere.  It's been working excellent for me as a fire spec mage.  Especially since I have all three with grappling abilities so I often have enemies running up to me... only to have them pulled into the Cass, Bull, Blackwall pile of doom.

I have yet to get into specializations, which may considerably change my party, but I have been doing well with the above on Nightmare.  I will probably spec myself as Cold/Spirit later on with Knight Enchanter.

I just use Cassandra Challenging everyone and putting up Shield Wall almost all of the time - except when whoever she is engaged with is paralysed or whatever.

Warriors get taken out in two hits, even with guard and such, so why not use ranged? (If warriors getting taken out in two hits isn't your experience, totally tell me. I'm still low level, but I'd like to use more of Cassandra. Actually, I think I just picked up Shield Bash last night. I'm sure I'll use that.)

My warriors are lasting more than two hits on Nightmare, but I have their tactics set so they use skills which build guard, and there are also two mages in the party casting Barrier a lot.  What I find is that there is a tipping point in each battle, especially rifts, where you just know you've lost the edge, or that you've tipped the balance in your favor.  And it always seems to revolve around taking out just the right number of mob enemies, or downing the Elites.

Also, I'm finding that it's important to pick your ground for the fight.  A few times I tried those veil rune locations that spawn all the terrors and undead.  I got hammered a lot, until I decided to prompt the event, then retreat to a more open area away from the trigger zone.  From there I tried to concentrate on locking down the terrors while the warriors pounded on them.  I also tend to save all my healing potions for these fights, and only use regen potions for topping up after a skirmish.
