It doesn't seem like this bug has been addressed on this thread (but I have seen it on other, unofficial threads regarding it):

Quest: The Trouble with Darkspawn

Bug/Trouble: On the last step in order to complete the quest, the player must "track down Servis in the Western Approach". However, once you track him down and kill him in combat, you are supposed to look at his body or something is what I am guessing. However, his body is not there. It's nowhere to be seen but there is a SUNBURST icon (on the local map) indicating where the body is so I can interact with it. Not possible. Nothing is there and the quest cannot be completed.

Minor bug: Dual-blade daggers (the aoe ones) don't count towards dagger weapons achievement in multiplayer (on PC, at least).

Playing on Xbox One and I killed all high dragons. The collections journal shows 10/10 killed, but my achievement for this still says I am only 90% of the way done with it and it failed to give me the achievement! Those fights are hard... I would Love to have my achievement for it. Thanks!

Cassandra, after mortally wounded, moves quickly to catch up with controlled character while escaping pack of bears.

After controlled character attacks bear that came with Cassandra and kills it, (they were so far away the computer moved them closer to me)

Cassandra falls to the ground in death, only to be regenerated with full HP.

It's like weekend at Bernie's only with wings and a female protagonist.

Druffalo owner buries himself up to the waist on a hill just outside of Redcliffe Farms Camp.  When Druffalo owner moves, he stands normally, but then falls back down into the imaginary hole.

*Invisible walls on areas that are normally passable - only when controlling the inquisitor

*Other sound effects becomes muted - this is frequent when casting the blizzard spell

*Resources that can't be collected

*Hitching - like a half a second the game would pause

*You get tossed high into the air when fighting dragons

Issues I've run into so far on PS4:

-Male Elf model broken in most (al?) armours, animations are broken.

-NPC or party members stuck in terrain.

-Falling through the world.

-NPC quest givers spawning "out of bounds". (fixed by reloading the area)

-NPC warping all over the place.

-Random NPC  showing up during cutscenes and clipping with the cast.

-Resources spawning under the world (unreachable).

-Viviene's bottom half of her armour casually walking across the screen in the background during a cutscene.

-Tactical view locked out/not working.

-Unable to switch characters during a mission (not scripted).

-Interacive cutscenes becoming unresponsive.

-Numerous clipping issues.

-Party members just standing around looking bored during a fight, unresponsive to commands unless played directly.

-Inquisitor pausing while running around, at intervals, mostly in Skyhold.

And then there is this little jewel:

I probably forgot one or two that didn't stand out as much or were only a one time thing.

Fire your game testers Bioware, they were obviously not doing their job, you're not Ubisoft... right?

p.s. Also fire the person who thought it was a good idea to put Jump and Use on the same button.

Ok, I'm on PC. Fairly middle ground for the specs Win 7 64bit I5 Quad 3.0Mhz 6gb Mem .....though my vid card sux. GTX 520......so I play at low graphic everything, and run it on lower resolution so I move normal. Worked fine with it.....no lag or nothing. No glorious scenery or nothing, but oh well. Needless to say I made it through the game without any major crappenings.
However some bugs that I want to point out.

At Haven or Skyhold my companions would not show. They were there, because I'd bump into them, but not visible. Certain ones would just suddenly appear if I stood there for about 30 seconds. Some never would. I would have to constantly go back and check to see if they were there. I just started a new game and it's still happening in Haven. Damned annoying when I am trying to keep everything caught up!!! Is it a graphical loading issue or what. Even knowing they are there I can't initiate talking to them. Believe me I've tried that. No visible bubble, no talk. I don't believe it fixed even with a cache clear or a restart. Sera and Solas are the only characters that never went invisible. Though Cullen rarely disappeared too at Skyhold. Haven he was gone most of the time. Once or twice. Blackwall was gone for much of the game. It's hard to get their quests when you can't SEE or TALK to them. So this was my biggest and most annoying BUG!!

Yeah the changing of the voice after getting Hawke......I think that is Universal.

The other huge annoyance was with the mouse. Text or icons would not always highlight to click on. Usually had to hit both mouse buttons at the same time when in the war room to click on the Orlais or Ferelden cards......or in dialog had to move into the center of the wheel and double button click around to get it to start highlighting. Had to be careful so I didn't hit the wrong text......which I did several times.

Sometimes running around, suddenly companions would start running amok crashing into me and knocking me out of the way. Seemed very buggy, but maybe programmers did that just to be tools.

I did have crashes to Windows several times. Those were usually going into the War Room or into a new area like a mansion or whatever. But they wouldn't constantly repeat. Restarted and moved on. So I don't know what causes that.

In this new game I had a bug I've not encountered before. A rift that would not spawn demons. Sat there like it was, but never did nor turned into the "solid" that you close with your hand. I had to run away from it several times, and come back for it to finally spit the demons out.

OH yeah. I stopped getting Perks to. After so many Perks.....I didn't count..........I stopped getting them. I got loads more influence but no more Perks. And there was a lot of stuff still to get. Third tier of Focus was sorely needed!!! :(

So there is my input. I'd say the mouse issue the invisible companions issue and the Perks were my worst bug problems. Sucks there are so many different ones, for different PCs. How to separate all that into what is our problem and what is yours is monumental. I'd guess 80% of issues on PC is the fault of the PC and not the game. Must be the worst time for the programmers...........after a game launch. Such a mountain of **** to sift through. I sympathize.

Into the Abyss keeps freezing during dialogue scenes. The amount of bugs in this game is beginning to become ridiculous, not even Dragon Age 2 was this bad when it launched.

I am literally incapable of getting past the dialogue, I cannot proceed with the game. Get it fixed soon, because I have put over 100 hundred hours into this game and I would like to finish it.

Logged in and came here simply to post this. I apologize about the length of the report, but I feel like I need to describe in detail what happened and how I replicated it.

I've encountered a bug 3 times now, the bug being I get a game over screen while characters are still alive. The "Your journey ends." screen comes up, even though at least one of my party members is alive and moving about. For me, all three times this has occurred have been in the same fight against the dragon in the Exalted Plains area, though I do not believe that it is anything specific to do with the boss fight that caused it. I believe it's just a bug within the game, it not recognizing that my character is indeed alive. The first time it happened, I blew it off as a non-reoccurring bug and said "these things happen." But when it happened a second time at a different point in the fight, I decided to test and see if i could reproduce what was going on, and I could. That was the third time it happened.

When three out of four party members are dead, if you attempt to revive another party member and the one doing the reviving dies at the same moment the revive process finishes, the revive will go through. It will complete and you can control your revived party member for several seconds. You can attack with them, move them, use abilities with them, and use them as though everything is normal. But after those few seconds have passed, for me it was variable between 5-15 seconds (or somewhere around there), the game over screen comes up even with one or more party members alive.

The first time it happened is my best example, and it was during my first attempt at the fight. I took the Herald, Cassandra, Solus, and Iron Bull into the fight against the dragon in the Exalted Plains. The fight was going well enough, until i got it down to a little less than half health. It killed the Herald and Cassandra, then blasted Solus with a lightning projectile attack all in quick succession. Iron Bull was the last one left alive, and I gave him long distance movement orders so that he could dodge the dragon's lightning projectiles while waiting for the after-effect on the ground of the one that hit Solus to fade. Once it did, I had Iron Bull revive Solus. Right as Solus came back to life, the dragon hit Iron Bull with one of the lightning projectiles. He died. I had Solus use Fade Jump to move away immediately, then cast revive on the area that Cassandra and my Herald died. The animation for their revival began and I saw their portraits in the left hand corner appear one more, along with their life bars. Then the game over screen appeared. Cassandra, Solus, and the Herald were all alive, yet I was interrupted by the game over screen and forced to reload.

It was understandably frustrating, and I do not know what in the game could cause it to register that all were dead and not alive, but it did so. The second time i fought the dragon I lost normally, but the third time the issue surfaced again, with me using the Herald to revive Solus after everyone else had died, and the Herald dying immediately after reviving him. Though that time I did not get the chance to cast the Revive spell using Solus, only the chance to move him using Fade Jump before the game over screen reared its ugly head. When I decided to test to see if I could duplicate it again, I did. I let my party stand still under the Hold Position order, let the dragon kill Iron Bull, the Herald, and Cassandra, and used Solus to just use the revive command (clicking on a party member's corpse), once he was at low health, on the Herald. The dragon attacked, Solus died, the Herald was successfully revived, and then game over. Interestingly, when I tested it a second time, making Solus use the Revive spell instead of the normal command, he died while casting the spell but successfully revived two party members and the fight went on without the game over screen.

I encourage anyone else who has the time or desire to test this and see if you can replicate it. If you can't, then hopefully it's just an issue affecting me and I will just have to live with it. If you can, then please for the love of god fix it lol. Been forced into a game over screen and having to reload when I am perfectly capable of continuing the fight, after having invested a good chunk of time into the fight itself is quite far beyond frustrating.

Unrelated: I've also noticed in my many attempts at fighting dragons that damaging all four legs until they bleed does not appear to always knock them onto the ground. In fact, it never does. Ever. Save once for me. I play on normal difficulty, so I've no clue if the mechanic is supposed to be removed from that difficulty, or if the mechanic is only present in the first dragon you may encounter in the Hinterlands area. But if that is the case, I see no point to having the legs be individually targetable. Many, many, many times have I ordered my party to attack individual legs until I see them bleed and hear that horrid sound the dragon makes. Many, many, many times have I had all four legs bleeding and the dragon still jumping around willy-nilly. No clue if this is a bug at all, but it is something i've noticed.

PC version

My skill tree bottom abilities are cutoff on my screen.  My mouse will stop at the bottom of my monitor and while I'm able to select new skills the bottom of the tree goes lower than my screen and can't select the final tiers of the tree.

All my graphics and screen size/resolution are set up correctly, no other issues with anything else like this.

when i attack a great bear some of his return attacks will not hurt my character which ruins the thrill of fighting a monstrous bear that suposibly frightens the toughest of chevaliers. The thing is the Bears stomp attack does massive damage like its suppose to but it's basic attack which is a paw swipe doesn't damage any of my characters like its suppose to. It's just an annoying bug that makes fighting great bears to easy. I like it when I'm afraid I'll be it's dinner lol

There are logic errors between active quest and complete quests that cause many gathering quests to burden the script editor with garbage "meaningless quest complete" or updates of completed quests when inventory changes.

Lotus and roots verified bug, others pending verification.

Floating bodies: We have discovered this bug when moving along in an area. Dead bodies from npc's that were laying around were floating in midair.

Floating Herbs: Same as the floating bodies. Herbs that are supposed to be on the ground are floating in midair.

Buried metal resources: Pressing the search button to see if there is anything around worth looting and then there is an outline of a metal resource but it's buried under rock and not lootable.

Non-lootable herbs: Came across this in a cave with Deep Mushrooms. There are deep mushrooms but they cannot be looted. We press the search button and the mushroom lights up but the loot button does not appear when looking at it. This is just where we came across the bug I am not sure if it is just centered on Deep Mushrooms or if it is for other lootable items.

Party Members taking damage when not in combat: This came up last night while playing. Varric while walking with the rest of the group says "Ouch" and then his health drops. We are not dropping from a cliff, we are just walking along a river on the Storm Coast and all of a sudden he says "Ouch" and loses some health. This also happened with Solas and a couple of other characters. Even the main character has had this happen once or twice.

Rogue Movement: We think this is a bug. I'm playing a elven dual wielding rogue and whenever I go to attack an enemy the enemy moves away but my rogue does not follow it and it is difficult to even keep a lock on the target that the rogue is fighting. If I switch to another party member like a archer or mage then the AI takes over and fights for me. However I find that little fun.

In Dragon Age 1 and 2 when we played as a rogue the rogue automatically would rush whatever oppoenent was targeted and fight endlessly. My girl and I think that should be patched in.

PC;

On quests where you have a bar that needs to be filled (collect so many of or kill so many of) the bars fill but often do not show as complete.

For the requisitions its not so bad as you can craft at the requisition table to progress.

But I have been playing on the hardest difficulty( which I often do for new games I really like) and I spent many hours trying to do the Unfinished Business quest and Romance Cassandra I killed the 5 targets out of 5 and the bar is full but now I can't finish this quest off and romance her. This is so frustrating as I have already put so many ours in trying to get to this stage.

- Biggest and worst one for me: No banters pre-Skyhold. No music in VR.

- The sound lags sometimes i.e when using Warrior charging abilities. The animation is too fast for the sound.

- After the ending fight in the Mage quest, the interface pops up instead of the choices and you have to skip all the cinematics to get around it.

- NPCs sometimes walk too far away when they start speaking to you, breaking the convo. If you run after them, sometimes this breaks the convo.

- Companions can't seem to decide where to stand sometimes, they run around in circles particularly during convos.

PS4

1) Conversations in some of the cutscenes have a tendency to lag for 2 or 3 minutes before the next dialogue response.
2) If playing for an extended time the sound starts to lag and disintegrate to the point that you have no choice but to stop playing the game (to be fair, this could be a PS4 system issue, but it only seems to happen when playing DAI. Usually happens once I've played 12 hours or more).
3) There is an invisible merchant at Skyhold that just showed up one day, although I can purchase items from said merchant.
4) Companions often refuse to follow to next area and often get stuck in the strangest places in the environment. They also sometimes do not Hold Position when ordered to, which can be annoying at times.
5) NPCs often walk into and/or merge with certain characters the Inquisitor is speaking to, such as a particular elf in the Skyhold garden when speaking to the Reverend Mother. Have also found a number of other NPC movements that are very irregular and intrusive.

PS4 (digital download)

- I'm not hearing any party banters at all.

- Audio glitch where sound effects have 3 sec delay.

- My save files got corrupted 3 times already.

- I'm also having the same dialogue box/ circle issue where i can't click on it.

- Loots sometimes appears next to me even though i didn't fight any enemies.

- Bears do not stop following you..EVER. Even if you disengage and run.

- Can't gather some of the crafting materials, because they are under the ground or dug deep inside a rock, lol.

- Sometimes, i can go through a wall of a house. So does my companions.

- NPC bumps at each other before they start a conversation.

I started a new game and went to hinterland only to find some red templars and fought them, LOL. Then Cassandra says Corypheus must've sent them...i haven't even gotten to that part of the story yet...so it kind of ruined the surprise for me, LOL. This is at the beginning when you first arrive in Hinterlands...

Got another bug when i use Flank attack with my rogue, it teleported me in the sky and at the very edge of Hinterland's map.

Companions takes a about 5 sec to follow my PC in combat. I'm not sure if this is a bug.

Got another bug where some of the shards disappeared on the map and i couldn't complete the quest.

There are instance where npc dialogues/ talking prevents you from looting a bag or a chest that is close to the npc(s). You have to reload and wait for them to finish talking, then the game permits you to loot that bag or chest.

While in combat, the sound fx disappears and sometimes its 7 sec delayed.

And these are all at the beginning area. I've re-started the game 6 times now, because of this, lol. I think i'm gonna stop playing until they release a patch.

Also, please get rid of that weird camera positioning when you target a specific enemy. It's not very helpful or dynamic.
