I'm stuck on 14/15 landmarks and I don't know what to do. The only place on the map I haven't been in is a cave in the southern part of the map where you fight the dragon which is blocked by a large rock.
I have no idea how to break it and REALLY want to know how to move it for completion of the map!!
You fight the dragon on the south of Nazaire's Pass Camp, there's no rock there. Concernig the Landmark post the ones you found(X's on the map), and I'll tell what you're missing, but I'm assuming it's the Ritual Tower.

There is no landmark in the cave blocked by that rock (which will be removed once you kill the dragon, but apparently resets if you leave the area afterwards). Here are the landmarks. Tell us which you're missing (eliminate the ones you've discovered by putting your cursor over the X for each claimed landmark on your map) and we can tell you where it is:

4. Zhores Tower
5. The Surveyor
6. Lamarr Tower
7. Shortcut
8. Shimmer Stone Mine
9. Ritual Tower
10. Lost Idol

16. The Thing in the Dark
If you have cleared the map (even without that cave), you should be able to check it to look for question marks or landmark icons (the orange triangles). TBH, though, I forget whether you have to be nearby at the time or just have passed it previously for those icons to show.
I'm also stuck at 14/15. Where is The Thing in the Dark and Shimmerstone Mine? I'm not seeing either of those X's on my map even though I only need 1 more landmark. I also have a landmark in the Southeast that won't tell me what it is named upon hovering over it.
You guys might be missing the same one I was. On your map, find the three flags that denote the areas where you had to go to the War Map to make a bridge over the poisonous areas. About right in the middle between the bottom and middle of the three flags is a cave (just to the right of a straight line between the two maps.)
The last landmark I was missing was in that cave.

The poisoned areas are cleared using war table missions. To get that mission, you need to go near where the green fog begins and click on the quest marker there.
I'm at 14/15 as well, but two are missing on my map: Lamarr Tower and Echo Back Fort (but I have the Echo Back Canyon Camp). And annoyingly there is no diamond letting me know where it is

You are brilliant! Once I came close to the tower in the area you indicated, two question marks showed up on the map. One was for the cave, the other for the landmark inside it.
I was able to get all the landmarks, though I noticed when I picked up the last one I needed, Ritual Tower, it seems to removes the quest from my quest log altogether. Much the same way that happens with Solas' quest, All New, Faded for Her upon completing it. I am assuming both are bugged that way.
I'm currently stuck at 24 / 25 Regions in the Approach discovered and I'm going insane. I've completed every side quest (to my knowledge), found every landmark, mosaic piece, bottle, and rift. The map is entirely open, but for the life of me I have no idea what didn't register as "discovered". The OCD completionist in me can't let this go until I have it done. Has anyone else run into this problem with areas not counting for discovery? I'm getting so sick of the sight of sand... 
EDIT: FOUND IT. Iska's Stand was the culprit -- midway between the Ritual Tower and Nazaire's Pass Camp, on an open spit of land with no markers. Very easy to miss.

I have a different situation: I found all the landmarks, but it's not registering as completed. Also, the markers for the quest "On The Chantry's Trail" are still on the map as exclamation marks. EA should REALLY fix bugs like this.
