I'm terrible at long posts, so I'll just leave a list of unique weapons & armor I come across and their locations.

??? = I don't remember where I got them. I'll comb through a second playthrough to find them, again.
One-handed:
 
The Sweetish Fingers - Hinterlands, Elite Templar mob during Templar's In the West.
 
Sword of Charris Allied - DLC

Axe of Green Edges - Fade Rift Beneath Crestwood

 

The Dueling Blade - Purchased from the merchant in Crestwood after completing the mayors quest.

 

Caliburn - Defeat Kaltenzahn (Emprise du Lion dragon)

 

Evanura - Emerald Graves, claimed Briathos' Tree and it appeared in a chest. Possible glitch, can't confirm.

 

Song of the Marshes -

 

Greats:

 

Gift of the Mountain Father - That Avaar that wants to murder you in Zombieland.

 

The Last Stand - Same zone, killed a random Avaar elite and it dropped.

 

Rebuke of the Sunderland - Emprise du Lion, in the tower near where you placed the note for the infected lover.

Siege's End - Purchase from your Skyhold merchant (the male).

 

Axe of the Dragon-Hunter - Purchasable from Bonnie Simms in Skyhold.

 

Render's Blade  - Giant drop in Forbidden Oasis.

 

Verdict - Found in hidden cache in Forbidden Oasis. To get to the cache, head southeast from the first inquisition encampment. You need to jump on the boards 4x to break thru to the chest.

Staves:

 

Wrath of Lovias - Pretty sure this was from the slaughter the Apostates in a cave quest in Hinterlands.

 

Staff of Aidahn Allied - DLC

 

Stormbreak - *I believe it was similar to Siege's End. Check your merchants once you have the perks unlocked.

 

Staff of Stasis - In Hushed Whispers.

 

Blizzard's Path - Chest at Echoback Fort where you fight Servis.

 

Tempest - Still Ruins, Western Approach (dungeon where time seems to have stopped).

 

Bloodwake - Do Sand and Ruin. This will lead you into the mountains and when you kill the Venatori Mage there at the end he'll drop this. Hissing Wastes.

 

Staff of Tylda Bright-Axe - Unlock all landmarks in the Hinterlands. Acquire in War Table mission afterward.

 

Staff of the Void - Hinerval in Emprise du Lion.

 

Daggers:

 

Blade of Tuhna Allied - DLC

 

Bleeder of Souls - In Hushed Whispers, Red Cliff Castle, explore the dungeon!

 

Silkdart - ???

 

Nameless Blade - Abyssal High Dragon in the Western Approach.

 

Gift of the Talons - ???

 

Nightslayer - Valeska's Watch in Emprise du Lion. Very short dungeon, lotta purples.

 

Promise of the Storm - Tomb of Fairel in the Hissing Wastes.

 

Bows:

Bane of Red Crossing - Check chests in Storm Coast (?)

 

The Trepanner's Requital - Winter's Palace.

 

Grunsmann's Bow - Purchasable from Bonnie Simms in Skyhold.

 

Longbow of the Griffon - Temple of Mythal after you talk with Abelas that ancient elf.

 

Farslayer - Chest in Exalted Plains. Beyond the bridge, North along the river.

 

Perseverance - Chat with the Chantry Priestess every time you run into her in the Hissing Wastes. I'll add a full guide once I get it myself. ;)

 

Shields:

Shield of the Emperor - Bought from vendors with merchant perks active. Full view

Wintersbreath - Valeska's Watch in Emprise du Lion. Very short dungeon, lotta purples. Full view

 

Magehunter - Buy from Bonnie in Skyhold. Full view

 

Dirthamen's Wisdom - A quest chain started in Shrine to Sylaise in Exalted Plains. Start by lighting a veilfire torch and collecting all the glyphs until you unlock a new area. Head back to Skyhold and go to "Lost Temple of Dirthamen" via the War Table. Once there, it's pretty straight forward. Follow the icons on the map and collect all the remains and read all of the glyphs (should you choose to). Once you collect everything, head to the main chamber and place all of the oogy stuff on their pedestals. Once that's done, a node will spawn, interact with it to spawn the boss. Boss spawns zombie adds, and uses ice attacks. Once it's dead, the door behind it will unlock and you can get your loot! Full view.

 

Aegis of the Order - Complete Crossing the Sulpher Springs War Table Ops to open access to the cave leading to the chest where this is.

 

Schaefer's Rampart - Located in the Storm Coast hidden in an unmarked cave. Get there by starting here and follow the crevice to the end here.

 

--- Armor ---

 

Light:

 

Ancient Elven Robes (elf req) - Possibly dropped from Elven Ruins in Exalted Plains.

 

High Keeper Robes (elf req) - Purchase at the Dalish Elf Camp on the Exalted Plains

 

 

 

 

Medium:

 

In Death, Sacrifice - Dragon in Crestwood.

 

 

 

 

 

 

Heavy:

 

Masterwork Vanguard Mail - Possibly looted from a chest or mobs in Western Approach.

 

Masterwork Vanguard Armor - Mercenary Fortress, Hinterlands

 

Masterwork Battlemaster Armor - Hinerval (possible random drop)

Siege's End can be bought w/o the perk. Just confirmed this. Costs 21k or some such.

Here's one that might vary by main character:

 

Staff of Tylda Bright-Axe (req. lvl 11): Unlock all landmarks in the Hinterlands. (I actually only had 15/17 - everything aside from the dragon-valley stuff) A mission called something like "locate the Axe of Tylda-Bright-Axe" with high completion time (~16h, depending on agents recruited) will open up on the War Table. I got a staff out of this, but upon completion the mission statement said something like: apparently "Axe" translates into "hafted weapon" - so I'm guessing this is a class-specific weapon based on your decision at the start of the game. 

 

 

Filling in some blanks / correcting some errors:

 

Staff of Stasis (req. lvl 6) - Loot from Defeating Alexius, In Hushed Whispers

Tempest (req. lvl 12) - Still Ruins, Western Approach (dungeon where time seems to have stopped)
 
Grunnsman's Bow - It might also drop in the Winter Palace, but it is -definitely- purchasable from Bonnie Simms in Skyhold.
 
 
Some ones you didn't have listed:
 
Magehunter (shield) - Purchasable from Bonnie Simms in Skyhold.
Axe of the Dragon-Hunter (greataxe) - Purchasable from Bonnie Simms in Skyhold.
Tenassarin (bow) - Purchaseable from the guy next to Bonnie Simms in Skyhold (same guy who sells Siege's End). 
 

 

Really nice work here so far though! Especially love the images so people can see the stats. (I'm playing on XB1 so I can't screenshot as nicely.)

Do you have anymore screenshots of Staff of the Void? I've seen it before but recall it being gold, whereas it looks darker in the image in this thread.

Man, I have a Bloodwake. I wish I could remember where I got it. It was recent so I'd say either out of the temple in the oasis, the orlisian court story quest or the maybe the knight's tomb in the Emerald Graves? Sorry, that's not much help.

There's a one-handed sword called Caliburn. I think I got it off a dragon.

Well, Bloodwake is not that all unique. Just got another. From a chest in the Hissing Wastes in the Venatori camp.

My source says look in the Intrinsic Pool. I'll have a look around myself.

 

Found it. Didn't drop the sword, so it's RNG.

The armor is actually called Sturdy Battlemaster Armor, the remaining pieces to complete the look come from as previously stated, a dragon. The gloves and leg pieces are crafted. Those are called: Battlemaster Arms & Battlemaster Legs. I think these came from the vendor in Hissing Wastes w/ the dog out in the canyon.

March of the Everlasting (Shield) (req. lvl 17) - +30% front defense, +27 maximum health, +8% melee defense, +4 strength, +5% damage for each enemy within 8 meters. - I really wanna say Abyssal High Dragon, but honestly not entirely sure where I got this.

I love how the ratio of unique weapons to unique armor is 20/1.  God this pisses me off.  Where is all the armor loot in this game?

Axe of Green Edges - Fade Rift Beneath Crestwood

 

You get 2 unique headgear pieces by completing the Winter Palace section of the main quest - which one(s) vary on who you side with there. It might be possible to get more, actually, since I hear there's a "reconciliation" option - which I didn't manage to get. You also get the Formal Attire armor schematic, which is the second-best light armor schematic I've found in ~40 hours - the only better one I've found so far are the Sturdy Battlemage schematics in the Hissing Wastes (which cost ~8k gold).

Dude I've found like... Maybe 5 or 6 unique armor sets. Out of the truckloads of unique weapons.
