I just returned from collecting all the shards in Forbidden oasis and a cut screen appeared when I returned to Skyhold. I havn't done many quests yet but Cole decided he wanted to leave and I couldn't stop him (different chat options). He had ALL my uber rogue gear with him.
Are any more party members due to leave at random times (so I can unequip their gear before they do this).
I feel robbed that he took all my good gear with him. Guess I wont be using rogues in the game any more.

What did you do to him? What did he say before leaving?
He's never wanted to leave my party; it's definitely not something that happens no matter what. For the others, if you've been constantly getting their disapproval or something, there's probably a chance they could leave. I don't know which ones will go and which one(s) will stay no matter what, though.

Is there an approval rating that you can see to see how well liked you are with each NPC, so I can try and please those that are low to prevent this from happening again.

Unfortunately, no, there's no way to tell other than listening to how they greet or interact with you. You can try to keep track of the "X disapproves/approves" notices, but that might get a little annoying.
There are nine potential companions; all of the quests to recruit them should either have their names on it or obviously be an attempt at getting your Inquisitor's attention. Which characters do you have out of these ones?: Vivienne, Sera, Iron Bull, Blackwall
At the least, you should still have Cassandra, Solas, Varric, and Dorian, but the other four need to be recruited. Vivienne and Sera's quests are picked up in Val Royeaux, one of Iron Bull's men should have told you where to find him (he's on the Storm Coast), and Blackwall is in the Hinterlands (Leliana should have told you about him).
Is there any way to get Blackwall after I missed him the first time around. I have a quest about a lone warden (NO LONGER AVAILABLE) in finished quests. Not sure how I missed him or maybe I did the quests in the wrong oder at the start. I know I tried to clear the first land of all sub quests then realised there was a main storyline I was supposed to be following.
I've started to talk to my NPC's now to try and get their personal quests. I was too busy enjoying myself with killing dragons before 
And yep I just got Dorian, I missed him too

If you have completed the Warden quest line at Adament than sorry NO. Once you clear everything up with the Wardens it doesn't make sense to go looking for a lone Warden to ask him why the Wardens are going missing. 
So yeah you get locked out of the mission, Sorry no Blackwall
