This bug removes Cullen (Skyhold) from the game forever.
After talking to Cullen about lyrium, he will eventually disappear from his office and be replaced with an NPC that gives you a quest that points you to where he and Cass are arguing.
However, if you were to try and talk to said NPC during Sera's quest (pranking advisors) you will be unable to interact with it anymore. The NPC will disappear, but reappear on reload. And despite reappearing, it will no longer be possible to talk to it therefore making it impossible to acquire the quest for Cullen's story progression.
I have tried saving and reloading, and going to a different map and coming back (works for some cases). Neither of these methods allow me to interact with the NPC.
