There seems to be a bug on the 15th landmark (The Last Step). It never showed up as one of the pyramid icons, only as a question mark on the map. When you reach that spot it changes to an X but never offered a flag to claim it, therefore, the collections journal entry doesn't count it and leaves you with only 14 of the 15 landmarks. I tried loading earlier save files and still got the same result all three times I attempted.

My game behaved the same way, but I did get all the landmarks completed anyway.
I saw your post before I came accross a spoiler map showing the locations, which made me realise I'd missed a cave in the middle of the map, where the gas was blocking it earlier, along the eastern mountain sides when standing in that area.
Maybe you've missed this too?

I have the same problem - I completed 14/15 Landmarks, all except the Last Step. Among these was the one with the cave torharry mentioned (it is called "The Thing in The Dark"). Now I'm stuck at 14/15, because everytime I get close to the Last Step, it changes to an X directly, giving me no opportunity to claim it.

 

This is an annoying bug and I hope it will get fixed soon!

I also can't get the Last Stand landmark. Or I think I can't. I can't tell for sure 'cause the Western Approach Landmarks entry is missing from my Journal.

 

Side note, there are 16 landmarks in the Western Approach total, not 15 as some of you are saying.

Ditto. Came looking online to see if that had happened to anyone else. Between the mosaics, landmarks, and the unlimited requisitions, this is an OCD nightmare. lol


Edit: Not soon after I found my last Landmark due west on a Tevinter tower.  You have to cross a small bridge. So I now have all landmarks for Western Approach.

Ok so this bug is killing me. Same thing happened. Went to get the last step but I couldn't claim jt. It just turned into an X when i got close. Now the game says I have 14/15 landmarks claimed but I have 15 X in the map. This is ridiculous.

the last step landmark is actually located in the tower area you fight Servis in, I remember finding it odd that it was listed in 2 places. Try going there, I did not have a landmark icon but saw the claim thing for it after I had the guards take Servis away.

Your lucky, my collections wont show Western Approach at all or register me claiming any of landmarks in top right of screen

even though claimed.

You can get the Last Step landmark by planting a flag, it's just that the flag you plant is not where the landmark appears. You plant the flag on the edge of the top floor of Echoback Fort, where you fight Servis. The site where the actual landmark appears is visible from the fort, but you have to walk over to both points to have the landmark display properly. I don't know if this is a bug or intended behavior.

I am with Senjougahara Hitagi, there are 16 landmarks not 15 thus when you count the X on your map you will count 15 but in the top right corner (Donno what it is called) it will tell you you have 14/15.

Bottom line is, look for one more landmark, in my case I missed "Thing in the dark", soon as I did zap quest completed.

I'dd like to think this is not a bug, just a trick by the developers to get us to scratch our collective heads a bit longer.
