The dungeon is accessible from outside in the courtyard. You can sort of see the entrance behind the Inquisitor in this screenshot:

I had a strange guy in my prison , someone I never judged , he asked me a riddle and then made wind sounds and said he was going to disappear.
He wasn't there when I went back.

That's not part of the game; that's lack of sleep. You've been playing too long!

Wait whoa whoa.  Strange riddling guy that makes wind sounds and disappear?  Could this be another appearance of Arl Foreshadow?

Otherwise yeah the guard says there no one when there's some people in there.

I found this guy hiding in a closet in my Inquisitor's quarters the first time I went up there... a bizarre but entertaining experience.  I wondered where he went, but I take it he moves around a lot?

 

And yeah... as far as the topic goes the guard seems to be glitched and never acknowledges new prisoners in my game, but they all seem to appear in the second room, the ruined area that is exposed to the mountains.  I don't know whether that's humane or not.  On the one hand they have a nice view, but on the other, they're fully exposed to the cold... :/
