I've heard that there a bug or some sort of trick to keep both of their loyalties and resolve their little cat fight with a paragon or renegade response. Only both options are greyed out. Exactly how does this work?

You simply need a high amount of renegade points to unlock the renegade response or a high amount of paragon points to unlock the paragon response. If it's important to you try loading a save before the mission which lead to the fight and instead do other missions to build up sufficient paragon or renegade points (it's best to only go for one).

I had a high paragon before and still couldn't choose the paragon option to resolve the fight. I imported a save from Mass Effect 1. I just completed Horizon and so far, recruited Garrus, Grunt, Jack, Mordin, and Zaeed. Loyalty missions didn't even pop up until I completed Horizon if that makes a difference.

Yeah loyalty missions only turn up after horizon. Again, I would just recommend trying to go back to before the fight, go on other missions (but not main missions), try to gain much more paragon points and then do the mission that leads to the fight, hopefully with enough paragon points to unlock the paragon option. You can also use points to upgrade your passive class power (Soldier: combart mastery, Engineer: Techmastery, etc) which increase your total paragon/renegade points. At the fourth upgrade level you get the choice between improving your stats for combat or increasing your paragon/renegade by lots. Go for the one which increase your paragon/renegade points.

If none of this works I'm afraid you're just going to choose between whose loyalty to keep and who's to lose.

Thats pretty much all the help I can give, hope it helps :) .

If I want both loyal, I do Miranda's loyalty mission first than talk with Jack and do her loyalty mission right away, then when it comes to picking whoever, they will both remain loyal.

One time I waited till later to do Jack's loyalty mission and ended up with only Miranda's loyalty and losing Jack's.

It's pretty strange, because I've always done Jack's loyalty mission almost at the end of game - maybe two missions before entering Omega 4 relay. I just wanted to have a high amount of paragon, to use a paragon response and keep both of their loyalties. I don't think it's some kind of bug or something like that. I finished Mass Effect 2 seven times - six times my all crew survived (once they didn't make it, because I played ME2 first time and I didn't even realise, that there will be such situation between Miranda and Jack). So...  as you can see it always worked - in my case at least.

I usually have a renagade response when facing both because I mostly play renegade. The few times I played paragon, I got the paragon response

The Intimidate requirements are lower than the Charm requirements.  It's also possible to regain the loyalty you've lost through later conversations; Charm or Intimidate are still required, but the requirements are the same (or possibly lower, definitely not higher).

There are other threads on here that discuss it at length, but if you haven't read them yet then be aware that your Charm and Intimidate options are not dependent only on your Paragon or Renegade score, but rather on the ratio of "score you actually have" / "highest score you could have had by now".

What BurningBlood said.

You can learn how charm/intimidate works here. (detailed explanation by dev on page 3)

Also this thread may help you more..

Have high enough paragon/renegade points.

Tip:Paragon actions are performed by clicking right mouse when option for Paragon action shows on screen while Renegade actions are done by clicking left mouse.Usually to raise paragon trough conversations I recommend hitting first responses in dialog-wheel if you wish paragon scores since second one is neutral and third choice at below is usually to build renegade points.Talk to as many people as you can so you can get paragon/renegade points,do also as many missions as possible.Once you have high enough paragon/renegade points certain dialogues depending on your reputation will unlock.

So that's that :) Once you have one of these reputations high enough you will have option to chose blue/red text in dialog-wheel that will get Miranda and Jack in line so you will have them both loyal(if Miranda or Jack are already p*ssed off at you then just go talk to them and if your reputation is high enough once again you will have unlocked dialog to make up with them and they will be loyal to you again).Try focusing on just being Paragon/Renegade instead of trying to be both.

Do Samara's loyalty mission before and use the Infinite Paragon Points exploit.
Another solution that works for me is postponing the mission that will start the confrontation until very late in the game, like just before acquiring the IFF.

Hey, man. I always trigger that argument just before the collectors raid the ship and take everyone because it costs a lot of paragon/renegade  points to keep Miranda and Jack happy. Remember, once the collectors raid your ship, that's it, mate. You have to do the final mission. If you can't keep both of them happy, don't use the one who is pissed off with you during the suicide mission because they'll die.

So do I normally and I find that works for me.

That particular paragon option is the hardest in the game. If I remember correctly, even if you go ME2 full paragon, it can happen that you don't have enough points. Play DLC's first, import a save from ME1 and do Jack's loyalty mission as late as possible. By then you should have enough paragon points.
Can't speak for renegade, though.

I'm pretty sure you can just side with one of them and come back later to regain the loyalty of the one you lost anyway.

Potentially.  In my first ME2 game I sided with Jack, and was never able to regain Miranda's loyalty afterward.
