I found him at second floor in in the ruins. He is lying right next to wooden barricades. He is easily overlooked.

This is extremely frustrating.  The first time I saw him stuck in a wall, now every time I go back to complete this quest I can hear him talking in the background but scoured whole zone and no body.  Of course getting support has been frustrating for this game.

I'm just adding my voice to this thread, as I have the same issue. I've gotten to Echoback Fort, but he's no where to be found. No voice, no floating body, nothing.

I'm adding my voice here as well (Playing on PS4). I hear my party members talk the same lines over and over (Cassandra saying this will improve our position, Varric saying that he would like to see Corypheus' face when he finds out, and Dorian saying one less bad Venatori)at one spot, but he's not there. I've searched all around the keep, and he isn't anywhere. Not a huge deal, but kinda annoying since this is my last outstanding quest in the Western Approach.

Yeah, this quest is broken/not broken.  Sometimes it works and sometimes it doesn't.  From what I've read, it's best to ensure you have a save before the start the quest that you can revert to as Servis will not show up in the final location periodically.  I have one playthrough in which I cannot complete this quest because of this bug. The marker keeps leading me to the fort where I fight the bruiser and the archers but no Servis.  He is just not there.  In my other playthrough I'm going to remember to create a hard save before starting this quest.

This quest does seem to be broken. After I killed Servias then picked "leave him there to die" but forgot to take care of the hole at that one shrine place but took care of it after fighting Servias this mission was still marked needing to be complete but every time I pass by the location Cassandra keeps saying how we cut down the threat against the darkspawn. On another character I did the hole first, defeated Servias and chose "take him him alive". This completed the quest.

Thanks to some of the posts here, I *finally* found him, laying up on the wall.  I had previously seen an enemy walking in the floor of the 2nd level and could not lock on or attack him, and was worried that I was stuck.  But thankfully after reading these, I found him laying injured.  Thanks!

Just keeping this alive, I've looked everywhere and he's still nowhere to be found. Bio ware please patch this!

I agree to it needing fixing! I have tried 4 times today, always reloading.  I hear mumbling before-hand but he's nowhere in sight. He's just not there. I like completing everything, so this is going to bother me now the whole game through.

I got this bug too. Same thing, No voice, no floating body, nothing.

 

After patch 3 I tried again, but nothing...

 

Well... I will continue the game, hoping that someday it will be patched.

I've got a solution for those of you who can see him but not interact with him lying on the ground at the top of the stairs. I had this same bug, as he was glitched partially through a wall and there seemed to be no prompt to examine him or speak with him. If this happens to you, enter your tactical view and target him, and it will give you the option to interact with him by pressing your action button (X in my case on PS4) and then edit the tactical view. Voila!

Hi guys I found a way to get serves to appear I was having the same problem with serves disappearing so I walked just outside the ruin saved it and returned to the menu and loaded back in there he was sitting on the battlements spoke to him and completed the quest

I see I'm not alone with this issue! I am playing on Xbox One and I ran into this road block. I fought my way to the Fort and killed everyone, but what I noticed is I travel with Dorian and he made Servis come back as a spirit, and then he faded away. Now I'm stuck with a quest indicator on the fort but Servis is dead for all I can tell, I even looted his body. 

 

Going back to Skyhold there is no one to judge. When I pass by the area now, one of the party members makes a comment about defeating him.

 

I've tried multiple tips posted here but none have worked. Hope a hotfix is applied soon, I like to have all my quests completed!

I'm having this problem, too! I can't see Servis anywhere. I've reloaded saves, tried kill the enemies in different orders, nothing makes him appear. I've read people see him floating sometimes, I've never seen him at all in the ruins. He's not there when I fight at all, I just see two archers and that big brute mofo. Am I missing something?

I have also just experienced this bug.  I tried reloading several times from nearby the fort, and on the 5th or 6th time I tried a different approach (the ramp / broken wall beside the fort) and still heard him stuck in the floor.  Then I tried the tactical view like AnotherCastle recommended, and I stopped hearing the voice.  Well, whatver.  I ran around, and lo and behold, there he was on the opposite wall limping on the ground.  No idea how it worked, or what I did exactly to make it work this time.

 

Anyway, I just wanted to post that in case someone else has a problem with this quest in the future and wants some possible solutions.

So I left the area because I couldn't find Servis and now that I've gone back I killed the enemies in the area but Servis isn't there anymore. There's a big guy that swings his hammer around but I don't think that's Servis. Is there any way that if you kill Servis but don't talk to him and then go back that you can still complete the quest?

I have the exact same issues! Quest remains active, mobs respawn, but no Servis.

Iv had the same problem except there is no Servis, when ever i go back it just the two archers and brute no mage. Anyone have any solutions?

Same problem here. No Servis. I have even took Varric in by himself in Stealth before any fighting and looked around and he wasn't there.

I have this quest left to do too, my last save is like 10 hours back so not reloading. I remember fighting him but he just died, it never gave me the chance to talk to him  after he was down he just despawned I guess and his not there any more and dont seem to respawn

If you have an earlier save, and you are on console, exit the game completely, clear your cache, and then replay from the earlier save. Sometimes it works.
