I like the idea of a spirit/demon for a romance (not like Cole, but a normal one). Solas' take on spirits was interesting and I think there is a lot of potential there.
Having the option to romance the villain could be really interesting. And I don't mean a villain character that's part of your team that you don't know is really a villain until the very end when they betray you or whatever. Meredith was a potentially great character that got squandered. She would have been perfect for this idea. Another similar one would work as well.
I would like a tough, Cassandra-like female warrior available for female characters that isn't a Qunari or Dwarf (too stereotypical on both of those, in my opinion). I'm really getting a little sick of all the female rogues and mages as being the only f/f options.

For the most part, the PC gets what the PC wants when it comes to romances.

I think it would be a good idea because it seems like a lot of work to have so many romance options. And if they would get rid of that they could put all of that work into the plot so that all of that content would be experienced by everyone instead of only having some of the content if you romanced cassandra.

They should just make the same amount of options for everyone. Lesbians have only two options as well, just like straight males. 2-3 romances no matter who you are would be best.

The other rogues only invite him to reunions. They really need to let fighters be archers again, and dual wield swords then we don't have problems like choir boys trying to act dastardly and failing badly.
A villainous romance. I would love one that was a mix of Stalker with a crush and Love makes you evil. Like Sarevok, but romanceable. I think several kinds of demons could potentially work at as a LI. If anything it could be someone that's attracted to the PC's power and not really anything else. Also romances that start with the LI going after you instead of the PC starting it.
Also an Avvar - I don't care about the gender or orientation. I just want an Avvar - doesn't even have to be a LI. I really want an Avvar party member. Also more party banter that talks about the romance would be great.
A male Ice Queen-type would be neat as well as a commoner male rogue. And bring back the brothels. I miss the Pearl and the Blooming Rose - they were hilarious. Also just more interaction that's related to the romance - like other characters talking about the couple, or something like the gossiping Orlesians in Skyhold.

A Set of complicated twins that you have to please both. Technically counting as one romance each, if you get them up high enough with certian conditions, should be able to have a dual romance, with benefits.

I'm not really sure being possessed by a vengeful spirit bent on mass murder actually qualifies as an illness. Now a badly written plot point, maybe.
