It wasn't blood in the bath. Fingers on the scalp, intimate, and then the moment is gone.
The game (or 'game' if you like) Gone Home. In-game, something things off the whole time, and then you see 'blood' in the bath, but the bottle next to it shows it is red hair dye. Sam dyed Lonnie's hair in the tub.
The whole house is dark and there's a storm outside so it feels like it is supposed to be scary, but it never is. (Well... unless you like to examine things and are easily startled...)

I *thought* this was Cortana from Halo saving a picture of John's smile when she's becoming fragmented, but the "her" throws me off... Maybe thinking of some other story with an AI that's facing deletion and saves a picture of its operator's smile?

It could be Halo (I'm more than certain it isn't, but it's a valid interpretation).
"She knows they're going to kill her" could be referring to Cortana being aware of her impending 'death', since 'smart' AIs in Halo go rampant after a period of 7 years, in which they literally think themselves to death. If not referring to this, it could also refer to her sacrifice at the end of Halo 4. This is probably the easiest line to interpret, since you visibly see and audibly hear Cortana descending into rampancy throughout Halo 3 and 4.
"Can only save what's important" would refer to the sacrifice, where 'what's important' would be Chief, where Cortana literally saves him from the Ur-Didact's grasp. Cortana tells Chief "They let me pick. Did I ever tell you that? Choose whichever Spartan I wanted." Chief would consider Cortana to be his closest friend throughout their time together, and Cortana reciprocated the feeling. Cortana knew that regardless of how the battle with the Ur-Didact turned out, she wouldn't live to see the end of it, so her last act in life was to break the Ur-Didact's grip on Chief, which allowed Chief to win.
"Precious space to remember her smile" could refer to Cortana's memory chip which Chief now keeps on a necklace as a keepsake. It is literally the "precious space to remember her smile" because it's the only piece of Cortana left in existence, and it's precious both because of its sentimental value, and because it's so small.

An old name burns inside = Anakin Skywalker thinking of himself as Anakin again instead of as Darth Vader
armor that shouldn't fit = the armor of Vader being grafted onto and into his body
lit by the faces of children he couldn't save = him thinking of his old self being due to Luke and Leia, the children he couldn't save despite him going to the Dark Side to try to save them and his wife

Like others, I'm going to say this is Blackwall because he wears armour and a name that's not his and he didn't realise that Lord Callier would be bringing his children along with him when he sent his men to kill him.
This does fit Blackwall pretty well, but I always get hung up on the final portion. To me, that line would seem to imply that Blackwall made some sort of attempt to save the children and that he failed rather than what really happened. That might just be me looking into it too deeply though.

Revan was a Sith Lord (He was their enemy the whole time) before Bastila and the Jedi Council decided to wipe his mind when they managed to capture him(but she made him forget), and game canon has him becoming a Jedi once again even after he regains his memories of being a Sith Lord later on (so he could change).
