Sorry folks, but it's actually another zanny glitch thread. 
Found our way from an old Chateau to this wonderful blocky desert world...I suppose that's what happens when you go deep into the Dales. (Also I fell into the map so I'm not featured in this picture. D: )
Want to guess what happens when you fall through Tervinter Ruin's floor?

Also had this happen to me on elven ruins once, minus the falling through the map. Was pretty bad, considering I was the legionnaire too >.<. Just held my shield up and prayed for the best.

This just happened to me last night. I was a legionnaire and my friend was a keeper. It was just the 2 of us. He could see everything just fine. I followed him and when enemies were close to me he'd tell me and I'd war cry, walking fortress and just kept swinging. That was zone 3. When he entered zone 4 I teleported back and was back in the map. Then it happened again on zone 5. We somehow survived the venitori commander and were successful, routine difficulty. Then my friend, the host, locked up and had to jump out. I then became the leader and was at the start of zone 5. He jumped back in and we beat em again!

Happened to be yesterday.
This happened on the first hours of multiplayer on my Xbox One, hasn't happened since. *Touches wood*.
I had a game happen where I was in a frozen tundra. The game was working fine for a second then all of a sudden I was dropped into a wasteland when one of my teammates went for the next zone. The next area I was dropped back into the regular map. I finished the zone (4), but as I approached Zone 5 I could see the wasteland again. When I teammates pressed forward I was dropped into that map again. I had to sit out the whole match stuck there. The game ended up getting frozen so I had to drop out. 
Happened when I dropped into my first DA MP match on launch night (the 18th for me). Haven't gotten it since.

Happened to me yesterday as well, in Tevinter Ruins :
On top of that, all ennemies were invisible after the second door, so we just stood around, waiting to die.
