From my experience, the only time hold position works is when the mage ordered to hold position starts getting his face eaten by a dragon....

The hold position function (or rather the hold button) is only a mockup in the PC version, placed there to make the game "appear" to be containing the functionality we expect from a Dragon Age game, which goes together with the "by PC gamers, for PC gamers" nonsense in order to get us to make the purchase.

Six months since launch and Bioware has been ignoring all feedback regarding this. Unlikely that it will ever be fixed. Probably too difficult for them - boohoo!

Radial menu Hold Position command doesn't work (don't know the commands on PC). Don't use it. Ever.

BUT double click on the ground where you want a party member to hold position and they will stay there until the currently controlled party member moves forward far enough. They will generally move somewhere pretty sensible when doing this and hold the new position. It's almost as good as the DA:O/DA2 method and I am a chronic Hold Position abuser. This is one of the most minor technical issues the game has.

EDIT: Also, disable tactics for close-quarters combat abilities on ranged characters - for example, disable Caltrops (only works in close-range) for Varric if he is set up as a ranged character. Otherwise ranged characters will enter melee areas. This is the only legitimately annoying part of holding position, and even then it's very simple to work around.

It happens whether you're in battle or not.  But, this is terrible behavior even when you're NOT in battle.  They have this one puzzle area, where you need to have different party members activate different levers and torches in order to unlock this one room.  You position one, tell everyone to hold position, change characters and then they all run back together again.  Very annoying.

If you tell your party to hold position, they should freaking do it until you tell them otherwise, load into new zone, or trigger a cutscene.
