The map has him in his secondary location (after everyone has chosen quarters) somewhere in the stables. I have been up and down in the stable and checked in corners in case he is having a PTSD issue. Did you find him easily? If so, where, please?

Save in front of his location, then load that save. Should show up. I've had that issue with him, and a few other companions. (Cole, Sera)

Yeah, Iron Bull occasionally disappears on me. Try saving and reloading as Jaron suggested.

It might be related to something, spoiler tagged just in case.

Part of his personal quest has him leave Skyhold. If he is an X on your party selection screen, then it's personal quest related. You can get him back, but first speak to the messenger that shows up where he should be (leave and revisit Skyhold for this, I think). Good luck, if this is the case.

 

Otherwise, it's probably the normal disappearing bug.
