There are many posts all over none of which with answers and many people all having the same issue. Does anyone here have all 21 regions explored in the exalted plains?
And for those who have not explored yet but still want to chime in YES this is with both operations unlocking both areas and still being 20/21.
Thought so I think everyone has this issue. I am also having this problem with an entirely explored western approach.
I have 20/21 in the plains, thats the only zone i do not have 100%, western approach is fine, and honestly, i havent tried that hard to get the 21/21 so i wouldnt be suprised if thats fine too... ill go take a look.. 
So... This is driving me up the wall at the moment. I've done everything there is to do in this area, and I'm also stuck on 20/21.

Same here. Did the puzzle, haven't killed the dragon, but I ran around the battlefield and still I'm stuck at 20/21. Western Approach is fine.

Yes, Western Approach is fine for me too. But I've run around everywhere in the plains that isn't blocked off by invisible walls, and can't get 21/21.
I've done the operation and crossed the bridge, and run around that area.
I've done the operation to the grove and run around that area.

I've done the big hand pit thing.
I've done the graveyard ruins.
I challenge anybody who says they DO have 21/21 to try and list the names of all 21 areas.
I'm going to run around and list them all later, just to see if maybe I've been in 21 areas and for some reason one of them just won't trigger the counter. Like in Black Flag, where the Kingston Viewpoints were stuck at 8/9 forever even though I did all 9 multiple times.
I have 21/21, but there are a couple of places that are "small" or "hard to trigger". Did you open the gates between the northern and southern parts for example? It's not a place where any quest takes you, and I imagine many people simply travel using the map once they're done in the northern part.

I assume you are talking about Western Approach here, rather than the Exalted Plains.
Okay people, because I am SO dedicated to the cause I spent hours this afternoon running around and not only listing the regions of the Exalted Plains, but also trying to draw them onto a map... it threw up something interesting.
Now, some of the boundaries aren't SUPER accurate, because there's some overlap and it depends which way you cross the border as to when the text pops up. However, I noticed a VERY LARGE, significant area in the middle of the map that does NOT trigger any change in region. Whichever region you enter it from, this area continues to say that is the region you are in. Indeed, by travelling only through this area, it was possible for me to run all the way from Fort Revasan to just outside the Northern Ramparts with the game still saying I was in "Exalted Plains: Fort Revasan" on the map screen. I suspect that this is the elusive "Area 21" and that the game has glitched so it doesn't trigger. I have outlined it in red on the map.
I wonder if anybody who says they DO have 21/21 in this area would be able to run into this area on their map and tell me what this region is called? Any chance of a dev speaking to the level designer for the Exalted Plains and getting some clarification?
I tweeted this to Mike Laidlaw who is out of office relaxing and not working on Dragon Age - oops, not my fault, twitter is upside down, I didn't get to that message yet! - but he said he'll pass it on. Hopefully if it is a glitch it is something that can be patched to affect saved games.
Also, bump for anyone who has 21/21 to PLEASE do a simple check of the area I outlined in red on the map.

Being that there is a rift in that area, if you look at the rift quests. It is labeled as Dirthamen's Grove
Ah, so it's a bug. This was absolutely maddening when I explored the Exalted Plains map. It's the only map where I haven't collected all regions. Thanks for your effort, Starscream!

So I wonder if "Dirthamen's Grove" is this red area, and the missing 21st Location?
It sure would be great if somebody who has 21/21 can check this out for us.

I have the name for this red region , but my games language it french ... so i don'T know how to say that... but in french it's ''Garnisons de la riviere'' ... so i supposed it's : ''Garrisons River'' ?

Wrong - as he has proven with save/reload. I did the same, and yes it is a no name region.
