I have explored everywhere in the Exalted Plains looking for this 21st region. I even pulled the trick of dismounting through the invisible walls in order to get through them. This allowed me to go to a lot of places that you wouldn't normally be able to go. I circled the entire map, as you can see from the map I've attached. Still no 21st region.
Well that sucks. Completionist nightmare.Someone found the 12th Mosaic through noclipping with cinematic tools, but it's behind a rockface, and they can't reach it.
I can not find the 21st area. Fort revasin teleporter is not showing up on my map as well. I have done the hand, the puzzles, the dragon etc.
Same for me, stuck at 20/21 and been around the map so many times... *sigh*

you need to go through the elven puzzle place. That gives you 21/21

The poster was referring to The Dead Hand, the underground chamber in the Exalted Plains that contains a large Elven Puzzle. It provides an alternate route through to the Crow Fens area, which contains the Exalted Plains Dragon.
It won't give you 21/21 though, since it's one of the 20 people have already got.

As far as I can see, the abandoned tent can only be reached with hopping fu and a bit of persistence. The last yard of the approach from the Eastern Ramparts has eluded me so far, and the approach from the north isn't the easiest either. Whether that is by design or due to unfortunate interaction between physics and display resolution is hard to tell... The placeables are unusual but non-interactive (apart from the lootable chest). This speaks for the hypothesis of deleted content but the poor reachability speaks against it...

The elusive Area 21 is just the middle of the map. But the game doesn't know the place. Did elves sabotage the files so they could hide their secret palace?
Okay people, because I am SO dedicated to the cause I spent hours this afternoon running around and not only listing the regions of the Exalted Plains, but also trying to draw them onto a map... it threw up something interesting.
Now, some of the boundaries aren't SUPER accurate, because there's some overlap and it depends which way you cross the border as to when the text pops up. However, I noticed a VERY LARGE, significant area in the middle of the map that does NOT trigger any change in region. Whichever region you enter it from, this area continues to say that is the region you are in. Indeed, by travelling only through this area, it was possible for me to run all the way from Fort Revasan to just outside the Northern Ramparts with the game still saying I was in "Exalted Plains: Fort Revasan" on the map screen. I suspect that this is the elusive "Area 21" and that the game has glitched so it doesn't trigger. I have outlined it in red on the map.
I wonder if anybody who says they DO have 21/21 in this area would be able to run into this area on their map and tell me what this region is called? Any chance of a dev speaking to the level designer for the Exalted Plains and getting some clarification?
Okay so im not sure if this has been found or not but i may have the name for the elusive area, "Pont Agur" which is stated in the war table (repair the bridge) and you are infact able to fast to the location from the war table. But once you are there, according to the map you aren't in a region, as pointed out in the red area by starscream above. I hope this helps!!

Definite bug. Agree with the above that #21 was likely intended to be Pont Agur, as the remaining items sound more like landmarks than general areas.
Personally, I'm interested in tracking down exactly where this "Offering to the Dread Wolf" is. There were statues all over the place, but I don't remember anything being particularly... blatant.
I had the same issue with Emerald Graves.
I'd done each landmark but it still only showed 20/21. I had a guide open alongside my game window that showed 21, I counted 21, but it only gave me 20.
Good thinking; however, I don't think it's going to be Pont Agur. "Pont Agur" means "Agur Bridge" in French, and it is the name of the bridge that you repair in the war table mission. The bridge is not included in the red nameless area indicated on the map posted above. I just discovered this as well. I don't think it's possible to get 21/21 with the game as it is. Some people have claimed it but I've never seen a screenshot or any proof of completion.

Whatever it turns out to be, I hope we get a solution soon. I agree, a completionist's nightmare! I hope they don't decide to just let this one go, since it is part of a quest, even though it's a small one. The squeaky wheel does get the grease.
Actually the bridge is within the red area. But again, it is possible the name is in the files just from the War Table thing. The other names in that list don't seem very Region-y though, so who knows?

It is a bugged which is probably triggered on completing certain progression I too have 20/21. The biggest lol was not being able to fall off the castle's top corner on the second island. Because that is where I thought it was, when seeing shapes and borders on that other island. Having got all the landmarks and done all the quests including secret mission locations. I also have a locked door in that zone that is open but showing on my map as stil locked for one of the towers in the north
The same thing happened with me in Crestwood on another playthrough. If I broke the dam before discovering the fisherman's hut. I wouldn't be credited with that zone afterwards, only the beach.
I haven't even bothered with the Hissing Waste regions because my horse keeps falling off the map when trying to look for them, because there are no borders.

This is just bugged. While exploring the Exalted Plains, I discovered all 21 areas just fine. Got the whole quest completed acknowledgement too. However, if you are looking at the "collections" in the journal it will still say 20/21.

Except travelling around the entire map, names only come up for 20 areas. Didn't get any quest acknowledgement. If you did, perhaps you could try going around to see what all 21 areas are? IMO though, it's not just the journal entry that's bugged.

You are my hero. Exalted Plains was driving me so nuts on my first playthrough, I gave up.

I have seen screenshots of 21/21, there is a link earlier in the thread to someone who has it. We just don't know how they have it, since it appears the majority can't get it (including me
