The "romance scenes" in the Witcher do make sense, in Geralt's perspective. He is a witcher, thus making him immune to all kinds of diseases, and he has been described as kind of a "ladies man" kind of person. He likes sex, quite frankly. If he is immune to the possible side effects, then why not? Witchers can't have children either so no risk of little Geralt's running around.
Even Triss has gotten used to Geralt being the way he is. I think their relationship is deeper than just sex, which is why they still deeply care for each other.
What I don't understand, is why so many women (in the Witcher 1, at least) would agree to have sex with him? Is he THAT attractive? Is it his deep voice? Or maybe the white hair? I really don't know.
Probably what you said no disease and no chance of having children with him also reputation.
