I recently got to Skyhold. I noticed that there's a question mark in the room, but I can't get to it because the only thing you can do in that room is hold a conference with the other people. So how do I see what the question mark is?

I think the question mark you're talking about is actually in your quarters, which are directly above the war room - go through the door closest to the throne to get there. The question mark should be a note on the desk in your quarters, unlocking the quest to upgrade Skyhold. This really confused me at first, because even though one is visually above the other they are both on the same level of the area map, so the markers will overlap.
