This just happened to me for the first time.
Conditions at time of shutdown:
-Blackwall in Party
-"Grandfather" ghost was aiding me at the time
-Was being attacked in the Avvar Blade Valley by a bear in front of a larger group of archers and other heavily defended enemies
-At least one other party member was dead already.
