In the "Wicked Eyes and Wicked Hearts" quest.

 

My party fought a bunch of harlequin.  ( I think?  Harlequin are to ones who disappear?)

 

The last one kept vanishing and appearing to attack me.

 

I could not lock on.  My party could not attack at all.

 

I finally killed him with AoE spells.
