Buy every upgrade for your Keep and then save the city. That means buying the wall upgrade, finding the granite for the dwarf responsible for the wall upgrade, finding all three ore samples for Herren and allowing him as many soldiers as he needs to guard the mines, sealing the entrance from the Deep Roads under Vigil's Keep, and possibly (since I don't know that this is mandatory) finding as much lyrium sand for Dworkin as he needs to make as many bombs as he wants. And I may be forgetting something else you need to do.
I've done all this yet it still shows the Keep getting destroyed on the map, then again that might just be game mechanic. I can't recall if your companions still die

Are you sure you got all of them? The first time you do so you get an Achievement. But yes, the game still assumes the Keep is destroyed until the epilogue even if you do everything right.

Its been a mix for me but more times then not my warden does what he/she can to save the city.
Because my warden does everything to build/repair and fortify Vigil's Keep, it usually survives but I do believe (bug or design) you can not return to the keep after saving the city. You only have one choice left and that's to go after the Mother and the Architect.
From a good wholesome character perspective. Save the city. But from the position of a strategist... Go back to the keep. Saving the towns and farms means nothing if the castle has fallen.

I saved Amaranthine after fully upgrading Vigil's Keep in order to send the message that the people come first.
Yet unfortunately, it fell anyway. Either way, the soldiers at Vigil's Keep knew the score when they signed on to the Wardens.

Well the thing is that fully building the Keep and equipping the men actually saves it. Sure it "falls" but it was not destroyed. Amaranthine even if you save it needed to be rebuilt. The end credit notes state it took a year to rebuild. And that the keep took 5 years to rebuild. Its much worse for the keep were you not prepared. I believe otherwise it was torn to the ground. And I believe your companions stand a chance of surviving, those that were left behind anyway.
