Hey there,

I have some locked door icons and quest icons that don't dissapear after unlocking the door or taking the quest, although there is no new quest.

 

Just to name 3:

 

Emprise du Lion

• Baron Edouard Desjardins at Suledin Keep, the icon moves with him, but he has no new quest
• a locked door icon of a tower door east of Suledin Keep, southwest the Tower Camp, although the door is unlocked already

 

Skyhold

• Undercroft chest, icon reappears everytime I reload Skyhold, but the chest is empty

Okay, this one I identified. If you rebuilt Judicael's Crossing before capturing Suledin Keep and before talking to the baron, the quest "They shall not pass" cannot be resolved. The quest says, you shall inspect the bridge to see if it can be repaired. You also get the quests Securing Safe Passage and Breeding Grounds from him.
