I too have heard the human man and dwarf woman in Val Royeaux talking with each other's voices. It's consistent.

I once had Josephine sinking into her chair during the conversation about the assassins coming after her until Leliana & the Inquisitor were speaking to a empty chair that spoke back with Josephine's head underneath the chair, then she randomly reappeared again at the end of the scene

I found that the guards in Skyhold are fond of teleporting, as well as running into walls, especially on the ramparts. Totally distracted me from my conversations with Hawke.

 

Also, I know this is probably a design decision, but I find it amusing you can't take fall damage in Skyhold. Once I discovered that, it made fast travel within the compound pretty useless.

During a cutscene, everyone had their lips glued together but they were still sort of moving. They all looked like their teeth fell out or something.

In Redcliffe after the lower half of Leliana's body walks around without the top half after he kneels down accepts defeat. Then in the fade when the envy demon assumes Leliana and slits Cullen's throat, I skipped through that and my character was running around with a sliced throat spewing spewing blood until the next cut scene.
