As far as warning points etc. I don't know but I know on the old board if you had a couple of bans in quick succession then you were at risk for a permaban. There are some instances like physical threats to other posters, devs, etc, pornography, generally douche baggery towards moderators etc that could result in a permaban immediately. Generally you would get a temp ban then the moderator would discuss your ban with a global moderator to see if it warranted a more stringent action. They have reversed a few permabans as well in some cases.
