You'll only have to avoid crestwood, the storm coast, the exalted plains and emerald graves and both of the western approach areas and the hidden oasis. All of which pop up in quick succession as soon as you've traveled to Orlais, which is just after Skyhold.
Other than the prologue (in which you see them very shortly, only a few seconds!) you can go a really really long time without encountering spiders at all. Just stay away from caves. Haven't encountered a spider out in the open yet, although I haven't explored all areas yet. Just saying, it's very possible to avoid those things for the most part.

They're in the hinterlands, and Not in a cave. They're in the region called witchwood.
I don't remember them being in witchwood, but it's always possible that I somehow walked around 'em without meaning to.

Me either i just finished 23 hours of pre skyhold stuff and didn't come across one and this is my second play through.

Interesting. Maybe I missed this part of witchwood, might have to go pay it another visit next time I'm in the hinterlands.
Anyway, point being: The first area is mostly spiderfree. You may apparently encounter them (or miss them, like ZipZap2000 and me), but they are definitely not a common enemy in the first area.

just stay away from caves if it's something that terrifies you.
You'll reach an area that can't really be avoided with a lot of spiders though...
But your quizzy can handle them and squish them. Or alternativley hide behind Mr Bull or Blackwall or Casandra

That was my first instance of them as well. I didn't encounter them in Witchwood until after reaching Skyhold.
u can't skip them because they are the main enemies in "Here lies your Abyss"
While i understand people with this phobia, i recommend you try and conquer your fear by encountering as many in-game spiders as you can.
This is the first step for you to overcome this phobia, killing virtual spiders that wont ultimately harm you in the real world (just think of them as CG graphics).
Once you get used to it, real life spiders might not be so terrifying.
Its easier said than done (I used to be afraid of cockroaches, until i played fallout 3... God, those Radroaches used to give me the chills) , but i hope you can enjoy the game and get over this phobia at the same time.

If you mean the freakingly huge variation of a usual spider, then, apparently, there is one. In Emerald Graves, during one of the side quests, in the open. Apparently, because I fell off the chair and can't confirm it was indeed the spider queen. My paranoid mind tells me it was.
There are a bunch of them in the Fade too. Was supposed to be about confronting fears in spider-form, too.
