That's pretty much my question. Her head icon makes her look like she's standing in a corner near where you catch the Iron Bull for the first while, but she's not there. I've found every other person great and small, but somehow I can't find Cassandra and no doubt she'll be somewhere obvious when it's pointed out.

She's near the training dummies. However there is a bug that sometimes party members are not in their usual spot. A save and reload fixes that.
To the right of and slightly behind the Tavern and left of the door leading to where the Blacksmiths set up shop. To her left you will find your Requisitions table thru the door you can't see in my pic, but is right next to the Quick Travel stone, where u can craft the Skyhold upgrades.

Yah, I had half the companions glitch out right when i needed to talk to all of them for a quest. Really looking forward to a patch.
