A) You entered the promo code in the wrong EA account, which for one reason or another is not linked to your xboxlive gamertag
Solution: Go to www.support.ea.com
Log in
Now, under the Support Links column on the left side of the page, click on the Email Us link.
Fill out the information, starting with console type [Xbox 360], game [Mass Effect], specific title
of game [Mass Effect 2], category of problem [Game Information], and specific problem
you are having [Promotions/Special Offers]
After selecting all these options from the drop down menus, you are presented with the option of either
emailing EA Support, or going into a live chat session with a EA customer service rep (CSR)
Select Live Chat, and wait for your turn in the que (took me a half hour cuz i was 30th in line)
Explain to the CSR that your gamertag is linked to the wrong EA Master Account, follow his instructions,
and the CSR should be able to re-link your gamertag to the proper Account.
This shouldnt take more than a half hour, depending on how many people are quequed for the chat sessions, which seems to be a lot. Its worth the wait though, and the CSRs are pretty helpful.
******In order to download the bonus items, your xboxlive gamertag must be linked to the EA Master Account that you redeem the code on.*****
I hope this helps everyone else who has been super confused about this whole process
