I think its cool to have a few members die, it adds realism to the whole thing. Members dieing could also significantly change the story in the next chapter.

As far as the reaper being activated at the end I think you misinterpreted it. What I gathered was that Harbinger was controlling the collectors through the head collector. Since the collector used to be prothean before the reapers modified I'm assuming the reapers somehow set them up as a back up in case something like, the ending of ME1 happened.
You would hope a group of ruthless, super intelligent AI's bent on harvesting the galaxy off and on for all eternity would have a back up plan beside flying in a straight line for x number of years to get back to the galaxy

Yeah, I hadn't noticed that either. that is REALLY REALLY cool.....wow, now I hope it happens, to see the entire galaxy, united like that, in a battle against the reapers....it'll be breathtakingly awesome.
on a side note, that human reaper scared the living daylights out of me. lol. I can't imagine seeing that fully constructed. that'd be something to fear.

Get all upgrades from your party members. Talk to them, investigate, and then go to UPGRADES (Legion does not have one).
Have Loyalty for all party members.
Take Miranda and Grunt with you at all times.
Send Tali through the pipe.

Not true. I did absolutely everything. Every side quest, 100% of the available systems explored, even the 4 clusters you can buy the maps of on Illium, and I still lost Mordin at the end. Miranda suggested a techie in the tunnels, and Tali was glued to my side, so I had Mordin go. What a mistake. 8(
