As per some of the online information I've found, these are the requirements:

 

"Completed Demands of the Qun and gained an alliance with the Qunari. Here Lies the Abyss or Wicked Eyes and Wicked Hearts complete."

 

So I finished Here Lies the Abyss, and then completed Demands of the Qun and chose to save the dreadnought. Still no operation available. The first one should be Failed Assassination Attempt Inquiry. Do I also have to do Wicked Eyes and Wicked Hearts? Trying to get the Masterwork Archon Staff schematic here!

Operations seem to be bugged as well as everything else. I took a look at the list on the wiki, and both my characters have not had a number of operations trigger. Especially my first playthrough where I'm level 14 Inquisition. My second playthrough I'm getting more of the general ones, but for example I haven't gotten the racial ones this time.

Eh just blitzed through Wicked Eyes and Wicked Hearts to see if that would trigger anything. Nope. Screw this, just going to go back to my save prior to making the final decision, going to go save the Chargers, it's what I felt better doing. The First Enchanter and Seer staves are better anyway.

I too am stuck at this point, my choices seem to "not" trigger "failed assassin attempt esquire" quest. Very frustrating, like many of the thing's in inquisition....

I got this war table op on my last playthrough. I can't say for certain what I did to trigger it, but I know that almost every time I entered the tavern, I would ask Iron Bull how things were going with the qunari alliance. I know that when you have him and the chargers, you have to ask Krem about jobs suitable for them in order to trigger the operations.

Also, if/when you get the quest to trigger, you have to select certain advisors, to keep the chain going. I don't recommend using the "recommended advisor." My suggestion would be to go to the wikia page and read the description for each operation, as it does a much better job of explain how to complete the chain successfully. I found this out the hard way, when the guide suggested I use Cullen to complete "Protect the Clan" for my elf inquisitor, and the operation ended. All I wanted was a new mount. Is that so much to ask? Lol

I have the same problem... There's just no "Failed assassination attempt inquiry" mission in my war table...

I've done the chain, not worth the Chargers. They have good war table operations too, and all you have to do is ask Krem. No farting around with the Ben Hassrah, making sure Lelieanna does this or that, etc.. Krem will lead the Charges with anyone making any effort to make it a bit easier on them and....

My fave is the one where Krem and crew set out to make people think there are more Inquisition forces than there are and march around with rattles on their legs and, light extra camp fires and stuff.
