Where do you get into them from? I came across a door in a cave that can't be opened from this side.

First you need to capture the Keep in Crestwood to gain access to the dam controls and drain the lake. Once that's done you'll have to go through Old Crestwood, which is south of the first camp, and you should find the cave entrance there.
