So in previous Bioware games, in particular Mass Effect, you could exhaust all party dialogue between main missions. This is, generally speaking, when the NPC's would be updated with new dialogue options for you to progress through.
DAI doesn't have this, per-se. Yes, new dialogue is unlocked after main missions, but it also seems to unlock just through playing the game and doing side content, other character's personal quests etc.
Example? I'd exhausted all of Iron Bull's dialogue, yet I randomly approached him after doing a handful of MMO-fetch quests and Josie's personal quest, only to find myself welcomed by a cinematic scene. The same applied to Cullen.
So, has anyone noticed what triggers these conversations? They were definitely exhausted up until I did the aforementioned content, and non of it was related to these specific characters.

Certain conversations are triggered by approval for example the ones were they tell they consider you to be friends and such. The wiki probably has the hard numbers.

Solas' special location talk with you is definitely triggered by approval, I had no more options with him, then took him to Crestwood with me to activate two artifacts, then next time at Skyhold it triggered. Which then caused his personal quest to trigger after that (hint if you see him sitting in the chair, it'll trigger a convo).

Advisors do not seem to have an approval per se. I was trying to figure it out for Cullen specifically, his dialogues (and scenes) had been so far obviously progressed by the 'big' missions. Within the 'chapter' it was sometimes revisiting the Skyhold, but more often just a chain of talks\scenes, that can be triggered one after the other. Once the 'chapter' part is done there won't be anything new.
Another factor is companion's quest. E.g. Varric had all the same talks before we did his, then he had some new options, and then - quiet again.
Minor-plot-sensitive things might exist, but I can't think of any. Some dialogues and quests are side-specific (mages vs templars).
Iron Bull's Storm Coast quest seems to be level-dependent.
But I wish we knew more about how is it intended to work, sometimes I'm afraid that arc progression might be bugged. E.g. I had no quests from Solas since that cave in Hinterlands. My approval must be good, and he's my constant party member that seems to like me.
I'm pro-templar, none-mage, Lv18, had done Adamant & shrine, the only zones with the undone 'big' area-quests are Exalted plains and du Lion, the main quest is going to be 'wicked eyes...' (once I get to it).
