I step on plates that I have not stepped on before but the whole combo shuts down. Am I missing something?

Are you doing the puzzle on all elevations? There's one of them that needs you to jump side ways out of the puzzle you'd complete to start another one close to it. If I had a picture it'd be easier to describe. There's also another one with a switch/lever you have to activate to resume the puzzle and it requires to come back to the beginning. The only thing I can tell you without giving you major spoilers (on how to complete them) would be to make sure that you zoom all the way out even if you end up in tactical view, because you're simply missing "part 2" of the puzzle that you think you've completed, just look around carefully.

You waited to long to complete the set. Two of the puzzles are two-parters, and you need to light up both platforms to complete the whole.

Ok one more time, I step on pieces that are NOT light up, but instead of lightning up the whole combo shuts down, this keeps happening over and over through the combo unpredictably, I cannot finish the sequence.

That's weird. Are you party members following you? They should stay away automatically I think (they did in my playthrough anyway). Although even if they did step on the puzzle floor I don't think that it'd count (so disregard this I guess). Anyway, if you're 100% certain that you're not stepping on the same piece/tile twice and the whole thing seemingly shuts down for no particular reason then... it's.... probably a bug? Try to reload? Not sure what to say. If you step on the tiles one time only and try to do the puzzle in "one straight line" (by that I mean just making sure that you don't step on the same one twice) then it should work. It's essentially the same principle as the Astrarium puzzles, but this time on the ground (I'm sure that you know that but I'm just mentioning it out of reflex, don't take this as an insult).
