Having just exited the Eluvian after the conversation with Flemeth, the location of the altar of mythal however is nowhere to be found on my world map, my inquisitors path quest category is now empty as well. I've tried going to the war room as well as speaking with Morrigan and Ic an't seem to get the location to appear. ( my inquisitor drank from the well, and was instructed to go fight the ancient dragon, but the location is not available on the map. Is there something I've missed?

I've gone back to an earlier save, and tried it letting morrigan drink, I met a similar problem where after meeting with flemeth the next quest in the line would not start. so upon exiting the eluvian my story quest catagory is just blank. there are no new operations in the war room nor are there any quest markers inside skyhold.

I found it!
The altar is not located on the war room map, it's on the world map southwest of the exalted plains.

I don't have it both on table and on the map. After we left the Fade quest just ended.

It is the same for me, there is no location either on the world map or war table, In addition it seems like there is a cutscene that is supposed to play of my character returning to skyhold from the temple of mythal, instead I just load back in the room with the eluvian, with my full party, everyone in their combat gear, and no active quest marker (I was able tog o the war table and get the cutscene talking about the red lyrium dragon, but unless my inquisitor drank from the well the quest just ends there. otherwise it sends me to meet with flemeth, and THEN it has the same problem as morrigan drinking where the main story quest just ends with no way to continue it.)
To clarify, in both versions, after following morrigan and keiran into the elluvian, and coming out, the main story quest ends/dissapears and no further progress is possible.

I believe I'm having a similar problem but i'm not really sure.

If Morrigan drinks you don't need to go to the altar (as Morrigan already has what you need), that only happens if you have the Inquisitor drink, so that's not a bug.

There is supposed to be a short scene of everyone running/falling out of the mirror after the temple, yes. I'm not sure why it isn't happening for you. I'm assuming you're on PC, I don't really know much about that as I'm on PS4 so this is a shot in the dark, but I seem to recall there was a cutscene frame rate command that forced cutscenes to run at 60fps but it messed with some cutscenes and could get people stuck in the opening Fade location instead of the opening cutscene taking place. I wonder if that is similar to this.
