Since moving to Skyhold I have found it increasingly difficult to locate companions to talk to them.   According to the map they should be in a particular place but when I get there I cannot see them.    A similar thing happened in Halamshiral when I wanted to talk to certain people (as indicated by the plot prompt) but they were not where they were meant to be.

 

I know people may move around a bit but some are where they are meant to be (Dorian, Josephine, Lelianna and Sera), some come and go (like Iron Bull, Cullen and Cole) but Cassandra I haven't seen since day one when she was training in the yard.  I think she is meant to be somewhere in the tavern but I haven't been able to find her.     Now Blackwall has gone missing.    What am I doing wrong?

Save and reload fixes it.

Been there done that still doesn't fix the problem. Now I did "Under her skin quest" and am supposed to report back to Leliana about what I found out at the temple and she is not in the tower where she is mapped as being at just an empty chair. I have even done other quests and came back and she is not there.

Sadly it's an all too common problem, not sure what causes it in particular. You can also try just running to another area (tavern, stables etc.) and back. Most of the time that's what has fixed it for me as I haven't had much luck with saving and reloading either.

She might be over by the statue in the little grotto up there. Cassandra is always near the door the the requisition table, usually working on the training dummies. Blackwall is in the barn by the stables unless hes out by the stump chopping wood, or he's been captured in VR and you didn't finish the quest to get him back. Cole is in the tavern attic, Iron Bull in the back of the first floor of the tavern, Sera second floor of tavern, in a room. Solas ground floor tower, Dorian in library, Lielianna Library attic. Cullen out the door with the red carpet from Solas's room then in the next small tower. Viv across the hall from war room door, up stairs. Jossie, on way to war room. Dagna and Herrit in the undercroft.

there are different levels to maps, and sometimes there are stages within levels, like viv looks like she is next to varric but is on a balcony. I guess your game could be bugged but I played a while before seeing the levels options

I also had some problems finding Leliana at one point when she was marked as being in the tower. I had somehow missed that the tower has quite more levels than you think and she was simply higher up. Getting there was done through stairs that were a nightmare to find. I felt completely lost. Keep looking higher up and you will eventually find her.
