How do you get this? I've heard people say that she gives it regardless of approval and I didn't ****** her off too much so why doesn't she give it?

How far are you into the game? I'm pretty sure she won't give until after you finish Wicked Hearts, or at least she waited that long for me both times I've played and I tend to be on good terms with her.

Wicked Hearts then. Right now I'm scouring the Hissing Wastes and Emprise du Leon for schematics.

It is difficult to tell whether it is approval-dependent or not, since the numbers are not displayed. Judging by my playthrough, it was approval-gated. I got her personal quest seconds after I judged someone and got positive approval from her. You can get significant approval if you find the three tomes she wants. Try getting those, and maybe then the personal quest will trigger.
If it is dependent on story progression, I got her quest after Wicked Hearts and Here Lies the Abyss. Good luck
