Well If things are going the way they are today, then I'll assume they are making the game first and then once that's finished, then they start work on the story and plot, well at least that's the way Bungie seemed to make Destiny.

BioWare is not being shy about discussing its hotly anticipated next generation space game MASS EFFECT 4 due out some time in the future for various systems to be played with a keyboard or controller. While it's true that they aren't saying much it is also the case that there are lots of things to consider. Recently employee Jos Hendriks discussed on the official forums for the upcoming computer video game not based around the adventures of Commander Shepard to be released at a later date that is not today nor tomorrow and neither is it two weeks ago that genetically modified and intensely trained ninja assassin moose may have a part in the extremely anticipated fourth installment in the successful Mass Effect universe, which will be made available to consumers at some point "ahead of the current point in time/space if we are to treat it from a linear perspective."
Although some fans were disappointed in the end of MASS EFFECT 3, released in 2012 for various systems, there is no denying that they are interested in hearing more about the next installment, and moose wiping out a small board room filled with office workers with just its left antler may indeed be a plot point in the upcoming supremely hyped game which will see the light of day from the netherworld of not-light eventually but not September 17th, 1976 unequivocally. Fans are passionate about the game but developers are watching demonstrations of moose to ensure it is as close to the source material as possible.

While I am disappointed by DAI, they have smartly given DAI it's time in the spotlight in 2014. I imagine we'll start to hear more about ME a few months into 2015, if the game is far enough along to justify any sort of peaks.

They are doing a good job of keeping things secret. I respect honest, trustworthy people. Who among BioWare employees would have anything to gain from leaks, anyway?

One thing for sure, the game is already in an advanced stage, I believe we won't even need to wait for E3 for any real new information, E3 will just rehash and compile things we will learn in the first half of 2015 and show us how this playes out in actual in-game footage.
So at the begining of 2015 - teaser.
March/April - first real info on the game.
May - more info.
June/July - we learn up to 90% of the stuff we want.

That's because if the leaker was to be caught, they won't only lose their job but they'd have an almost impossible time finding another job in the video game industry, let alone any other job that involves NDA's. That wouldn't be worth the risk if you ask me.
Oh, and as others have said, they're all indoctrinated.
