Where is Skyhold? I am at level 13 and no clue....I did the Hinderlands, marsh area, the storm coast, and now working on the tedious Forbidden Orlais (not my fav part though looks nice)

Did I just read Forbidden Orlais? XDD.

comments on the whole Warden thing plus some stuff specific to the world state. I also try to do the Inner Circle quests right after getting Skyhold, but it's just a personal preference.

I tend to wrap up everything left in the Hinterlands , Storm Coast , Fallow Mire and a bit of the Oasis.
Then Crestwood , western approach ,Here Lies the Abyss.
Exalted Plains , Wicked Heart.
Emerald Graves , Emprise du Lion , and the last desert zone.

Should the Exalted Plains definitely be completed before Wicked Heart? Will parts of that disappear?

I don't think anything disappears from the optional areas, but it kinda makes sense story-wise to do EP first since it gives you some context for WEWH (not much, though, and it's actually pretty interesting to visit after WEWH, as well).

I'd say visit it both times, I did it after WEWH and was totally confused as to what was going on because they acted like something was still going on, and then went to oh ya that happened.
 
Since we are talking about areas, has anyone gotten approval from Varric for killing enemies in The Lion?  I figured he would since he likes it when you kill
