DW rogue is not for the AI. Either respec him to archer (Archer Assassin is very good), use Sera or Varric instead or micromanage him extensively.
Sadly there's no way out of it since DW rogue is the most powerful class in the game when it comes to damage but this comes at the price of required babysitting. Given this the class is a good choice for the PC, and a bad one for an AI companion (if you play on hard or higher; on nightmare simply forget about it).

You don't have to if you don't want. As I said you can simply respec him to an archer if you don't want to use Sera. Archer with Assassin spec is a very good synergy, some say even better than Archer Tempest.

I found the best way to keep him alive was to leave him in Skyhold. He rarely got hit and wasn't chugging healing potions every 2 minutes.
Seriously, why is Varric useless? Unless you arent keeping up with Bianca's upgrading he is as useful as Sera.

That's the problem. I WOULD upgrade Bianca but coming across the right upgrades is like trying to find a bride in a brothel. BioWare does a damn good job at keeping em scarce.

The only way you can try to have him as a DW rogue on hard or above is by giving him +guard items. At the beginning when you have only +3 guard you will have to micromanage him a little (on hard probably it is fine already at +3) but when you get +5 guard then you can mix the +5 and +3 for +8 and from that point on you can just leave him be and he will not die - or very seldomly do - either on nightmare (still he will never be as good as a DW rogue used by the player).
However in doing so you remove the possibility from yourself on using those same masterworks, so depending on your build it can be appropriate or not.

The fun part is that enemy AI assassins, instead, are very good in the game. They always stealth and go for backstabbing the weaker party members and they are very good at using evading abilities to avoid taking damage.
Then you have someone like Cole that never seems to use stealth in the appropriate way for his spec and/or to pick targets of opportunity or either to simply try to flank correctly. If Cole (but also other party members) were even half as good as their AI enemy classes counterparts (bar mages, that for enemies just buff the others) the party AI would be really good in the game. So damn strange, really; if Bioware could do it with enemy AI I cannot understand why they haven't done the same for the party. Bah.
Haven't used Cole much yet, but am I right in thinking Hidden Blades actually does more damage with a bow? Because it's based off of weapon damage, and presumably that's not calculated for dual wield by adding both daggers together... or is it?
That said, Hidden Blades does kind of imply... "blades"... same goes for bow Sera with thousand cuts.

The Skyhold vendor carries the schematics for the crossbow, increasing the quality of the schematics available as you progress through the main storyline.
Just like the weapons and armour you can buy in stores are crap compared to what you can craft yourself, the upgrades for weapons and armour that are sold are crap compared to the upgrades you can craft. That also goes for Bianca. So buy those Bianca schematics at Skyhold and, as you progress through the main quest, buy better ones and craft better upgrades.
And make sure to make the most out of Varric by making him a crit% monster via armour- and Bianca-upgrades such that he gains 10 stamina on nearly every shot. Since Varric has the Artificer class, he is more stamina limited than cooldown limited so long as one or more of your other party members have decent crit%, and can burn as much stamina as he can get, and since all classes have options for good crit-based builds, you probably don't have to go much out of the way to make at least one of the others in the team help out Varric with crits, just like Varric helps everybody else.
Must be a bug on my end then, because the store only ever stocks tier one Bianca upgrades.
I've found a good number out in the field, but it's still an underwhelming weapon.
Two of the vendors at Skyhold have Bianca schematics upgrades.
Bonny Sims get ever better Bianca upgrade schematics (and upgrades, but the schematics are better) as you advance in the main quest and complete the quests and war missions that are tied to the trade hubs. When you talk to her she'll talk about how many trade hubs you've liberated etc. and she'll occasionally give you a mission. I don't know exactly which quests triggers what, as I haven't been paying close attention.
Her silent partner next to the fast travel stone sells tier 1 Bianca schematics.

Give him Parry, upgrade his Stealth, and also give him all the upgraded abilities that also drop him into stealth. Cole should be in and out of stealth all the time.
The AI can't play rogues worth a damn.
If you insist on bringing a rogue make it an archer and ideally make it be sera and set flask of lightning to preferred that way she can just chug that and auto attack like a machine gun.
