Since companions' approvals cannot be viewed, the only indicator for when a companion has become a friend appears to be cutscenes. Some are fairly obvious, while others are not so. The friendship cutscene is also not necessarily the cutscene after a companion's quest completion.

 

Here is what I've noted thus far for various companions' friendship cutscenes, or at least I believe that they are the friendship cutscenes. Please let me know what cutscenes I've missed thus far so I can update the thread.

 

Blackwall:

Unknown; only just finished the companion quest myself, and no new cutscene is showing up.

 

Cassandra:

Ooh, didn't get Vivienne and Sera's, though not surprising. Still feels unclear when you have high approval with the others.
