OK, done some digging now.
It seems all updates are placed in a "patch"-folder under "Dragon Age Inquisition" - "Updates" - "Patch". I suspect the game is told to use the files in this folder over the files in the base structure of the folders. In the folder (deep breath) "Update" - "Patch" - "Data" - "Win32" - "da3" - "configurations" - "gameconfigurations" (phew) there are two updated files, which I guess the game is now using.
Over to the speculation now. I'm guessing these game configuration files are set to Low or Medium (as per normal in a patch), and I suspect that fiddling about with the in-game configuration options still only change the files in the base structure of the folders, and not the files in the "Update" - "Patch" structure. If so, deleting the "gameconfigurations"-folder, in the "patch"-tree might fix this. Or break the game entirely - I'm not an engineer. Try it at your own risk.
If any of you will try this, do not actually delete file(s), just move them elsewhere (on your desktop for example).
Even if deleted, you can still return them by verify option through Origin client, but still... It's easier to move them back.
And please post if removing file(s) helped!

Looks like BioWare skipped QA and decided that their customers will test it instead.
Like we have been doing since the day 1.
Glad to know I'm not the only one noticing this. I'm chalking this patch up as a prank on all PC gamers. Since PC's are capable of rendering the game at the best settings, someone at HQ thought it would be funny to make us see what it feels like to own a 360, to make us feel the sorrows of all console players with all these low graphics and whatnot, and in a way to make us understand their pain, and bring us all closer together as a community. That's how I see it anyway,cuz I can't for the life of me think of a reason how "Lowering the graphics quality, to make the game run better" would be seen as a legitimate "patch," or maybe someone just dun goofed. Either way, I'll hold off from playing till this is hotfixed, just like I was doing before this patch. lol It's like I'm goin around in circles.

Water still looks the same, well at least for me. Water set to high.
Probably because the water uses a different shader than hair and ground...?
OMG please hotfix the hot mess of shine and ugly that is forcing us all to play on low settings.
I have a powerful PC for a reason, that reason is, I am a graphics snob. This is horrible. Painful. It makes me cry inside.

i guess best idea is to return to the game in 6 months, i believe they will fix this mess till then. So i finally have time to play through Jade Empire. Also with latest patch it has similar to DAI graphics and i will not be feeling like i miss something =)

Well, so deleting the configurations folder from the patch-folder did exactly nothing.
Which is even weirder, tbh. The game seems to be looking in the wrong place for the config files. Perhaps a less easy fix than I suspected in the first place.

Tried, doesn't break the game but doesn't fix anything to me.
Am I the only one getting crashes while meeting Varric's mysterious friend? I tought they would fix it..
