Warden Mistakenly Listed as Dead

So I finally begun playing "Dragon Age: Inquisition", I've made it through the beginning and I'm currently scouting the area around the Inquisition camp in Haven. I did a few quests, and I got a codex entry about the Fifth Blight...

 

Saying the Warden died fighting the Archedemon. That's not what happened in my playthrough, and it's not what I inputted into the Keep. The Keep says my Warden is "Alive and Well". That's an enormously important detail, and if the game got even it wrong I don't want to keep playing it until I find out how to fix it. For all other intents, the game followed my Tapestry correctly so far, so it's not that I forgot to upload it or anything.

 

1. What is the meaning of this bug?

2. How do I resolve this issue?

3. Can I resolve this issue without having to restart the game? Having to do it at this point would be both incredibly disappointing and really kill all the fun and excitment of this first foray into the game.

 

I've waited for this game for years, and I spent the last several months finishing all the Dragon Age games so far again so that I can start playing in the world I created. If I can't have that, than I wasted my money, time and emotional energy on Dragon Age: Inquisiton.
