Same for me. I just assume it's bugged. Pretty safe bet for this game when things don't happen the way I expect them to.

I thinks it's indeed a bug. I killed 10 dragons and didn't get the achievement (on Origin)

OP watch the BBC Pride and Prejudice and learn how they talk. It will help in knowing which is the right dialogue choice to gain approval, I got 97 approval as an elf.
Lil tells you what you need to do, the same with Josie, Be as vuage as possible. Thats all you need to do.
What i hated was the god damn platforming... BW, just because you have the jump button, doesn't mean you need a jumping puzzle... the controls where bad and you didn't give enough room for errors in your own engine and control,
You did it fine in your MMO.

It's not bugged, but the description is misleading. I'm pretty sure it's >85 approval and you need to shame Florianne in front of the court i.e. not kill her.

Yeah, because playing the game has worked great for Orlais so far. Civil war, Cory using their own stupidity against them..yep..that whole game thing..awesome.
You are not looking at the big picture, instead choosing to focus purely on one distinct moment in "the now" of Orlais's long existence. The Game is part of a natural set of checks and balances that allows Orlais to thrive both militarily and economically without devolving into outright violence everytime there's a conflict between powerful individuals. The Game has served Orlais well throughout its history and because it works, it has allowed Orlais to consolidate its resources to become "The most powerful human nation in all of Thedas", as the many codices and game promo information has described it.
Corypheus is an incredibly dangerous threat, yes. And think about the fact that without such a nation as the Orlesian Empire, which is quite possibly the only one strong enough to outright oppose his machinations, his plans would go off even more easily in a fragmented collection of smaller kingdoms such as the ones that compose the rest of Southern Thedas. In fact, if you're canny enough to have realized this already, that's why Corypheus is plotting against Orlais and trying to destabilize it.
With such a long history of stability and growth, Orlais is incredibly lucky in the scheme of things to have only encountered a few stumbling blocks such as the current Civil War. It's the bad timing of coincidence (or is it?) that the Civil War and Briala's spat take place at the same time as Corypheus's rise to power. Coupled with the weakening of the Chantry due to the Mage-Templar conflict, it's really preferable that Orlais NOT devolve into even wider violence than it already has. The Chevaliers actually did a decent job of confining as much of their fighting as possible to a few key areas, if you examine their campaign.
The Game is essentially the real-life truism that politics, even faulty bad politics, is generally cheaper (in both human and resource costs), more humane, and preferable to open warfare. You spare the people, the national infrastructure, and the economy the devastation that would come through war, by sacrificing the occasional pawn, risking temporary loss of dignity and reputation. and maybe a few inches somewhere on a map's borders that moves back and forth over the years.
It sounds brave and courageous to propose every potential conflict be pounded out honestly, through direct words and action, but in practicality all those accomplish is the death and misery of many people for the sake of boldness. The Game exists to diffuse the natural tendencies of conflict and channel them into something less bloody and destructive.

Never play Origins. Anyone care to explain to me what was so infamous about this fade quest in hearing so much about?
In Origin's Circle Tower quest, a sloth demon sends you and your party to the fade.
Your party is split up;you are alone, and must traverse the Fade by finding abilities to transform into different creatures so you can enter inaccessible areas. All this while there are enemies(including bosses/sub-bosses) to fight.
Eventually you run into your party members(individually), band together, and escape.
This can take a long time for someone who doesn't know where to go for which ability, when you need them for what doorways/paths, and it is very easy to get lost.
Also, the Fade's aesthetics are very muted;the color palette is uncomfortable(by design).
New players got stuck there for hours. Some were stuck for days just unable to fathom how to progress.
I remember my stepbrother going crazy asking me how to get past it because he struggled to figure things out.
So The Fade sequence of Origins became notorious for being a very hated part of the game. It is why the sequences in the Fade of later games were either shorter(DA2) or more railroaded(Inquisition).

The only thing I disliked about it was the "timer" that caused you to lose reputation with the Orlesian court if you wandered off for too long. It went completely against the entire purpose of the game (ie. open ended exploration at your own pace) and made me feel like I had to rush through the various "off limits" areas.
Yeah. I hate against-the-clock quests nearly as much as I hate escort quests, but other than that annoyance, I actually enjoyed it a lot.

I liked it. It was a nice change of pace. Made me hate Orlesian politics, but the mission was quite fun!
The DAO fade level however, ugh. Completed it twice, can't bring myself to do it again. I actually have a DAO save just sitting there at the circle tower, half way through the mission and couldn't bring myself to continue. I boot it up every so often when I'm feeling nostalgic, remember where when my last save is, switch it back off.

Dialogue traps, a timed dungeon and I'm forced to play "their" game "their" way (stupid annoying Orlesians!). I never had any issue with any fade levels in DAO but this has to be the brain child of someone whom just enjoys the implementation of a section of gameplay that operates as complete reversal of the established gameplay where you have go at your own pace exploration and dialogue options that don't result in utter failure. I mean your character's dialogue cannot be a true representation of him/herself which makes the whole thing beyond annoying. Heck I just walked in and lost 10 pts for being a Dwarf.
I've done it two times now, I even got to 100 -- where is my achievement?! *shakes fist* -- but this next time I will probably hold off as long as possible. I think it's the most tedious part of the whole game. However much I dislike it, I do think that the structure of the mission made sense, including the timed aspects. I think the main problem lies in the fact that this is the only portion of the game to operate in this way, having a high potential to confuse and/or frustrate the player.
Keep in mind that you (most likely) have the people that were a fan of the non-combat aspects of the Mark of the Assassin DLC to thank for this. I recall there being a lot of forum discussion regarding it at the time, as well as a lot of back and forth between players and developers. Those scenarios were a test for them, and similar types of play were clearly implemented, and greatly expanded on, for this game.

Got 100% as an Elf, but Really bothered that they didn't let you go back at a later point, I'm still curious as to the point of the veil fire in the library...
But what's really grinding my gears;
1. 20/21 exulted plains locations
2. 10/12 Mosaic The Fall
3. 10/10 High dragons dead and no achievement, its the only achievement I don't have and I have no previous save(before dragon kill) so if I want it I need to do another play through.
Bugs...

yeah I was pissed too when I first did it. I got -10 court approval because I was a mage.... And all the stupid time contraints while exploring and the amount of collecting backtracking quests are really bad.

I really enjoyed this questline. Had to play along with snob and racist Orlesian nobles according their own rules, but all I wanted is to to kick them till they reach the Temple of Sacred Ashes to face Cory alone.
I laughed so hard when I saw poor Cullen being "under siege" by Orlesian men and women (one of them actually touches his butt, lol)- and after this quest we get some funny moments at the war table as well:)

Lol imagine the Duchess hitting you with a sonic boom.
My only major complaints are the terrible formal attire and the fact that you get an Orlesian mask but can never equip the damn thing. It's baffling.
Yeah the Inquisition members all wearing the same outfit and having no masks was...odd. I feel like they could have at least slapped masks on everyone and given men and women different outfits.

How do you finish with 100? 100 is easy to get before the final part of the mission, but I always lose a few points in the royal wing. Even if you rush through this section, I can't imagine not losing at least one or two.

Isn't Orlais the base of the Chantry as well as the place where the mage rebellion effectively started ? Also with the mage/templar war only recently ended it's not uncalled for to get a minus 10 approval with the court for being a mage. Also I don't know the relevance of the whole court approval thing aside from the achievement I never finished that part with approval lower than 85 but I suspect you would get the cooperation of Orlais despite not having the court charmed.
Finally nothing beats telling the 3 potential rulers or Orlais that they now work for you essentially making you the ruler of Orlais in all but name.
EDIT: As mentioned above the achievement description is not accurate. You dont need to finish the whole part with 100 approval but to confront the Duchess at the end instead of fighting her. That way also makes a really funny outcome on the judgement of the Duchess later on. Problem with that way is that Celene will be the Empress unless you make all 3 of them work for you so if you want Gaspard sadly you can't take the trophy or if you can I have yet to find a way.

Install the mod "Skip the Fade".
It's simple and easy, you skip the Fade (well....duh) and you keep the stat points you were going to gain there.

My only disappointments about this quest were:
-No option to wear a pretty dress if I wanted to.
-I wasn't wearing a dang mask in my cut scenes WHEN I SHOULD'VE BEEN
-Vivenne was stuck in the doofy dress uniform, maskless. Why would I ever do that to her?
