Yeah some party members can and will leave, but most of them give YOU the option to kick them out if their approval of you is low.
Cole & Dorian will just bounce on you if you ****** them off enough, whereas Sera will just threaten to leave but you can convince her to stay.
It's not as serious as in DAO or DA2 though. In those games, you really felt the danger of people abandoning you. I wish it was a combination of Mass Effect 2 & and this DAO where some party members (the optional recruits) would leave if the approval was too low, but also that EVERYONE has some special "loyalty" ability/passive that would only be available if their approval was high enough.
What does approval really do? Does it matter?

I kind of wish it was easier to get companions mad at you, so that the all the + - approval would inevitably make someone leave you due to low approval. I have more companions than I need, and I always want more choice consequences.

Not only leave, but getting them to actively oppose you would be interesting.
