Has anyone had anything weird happen with this quest?

 

I finished it while in Haven, but now every time I use up all my Elfroots making grenades and potions, then collect more Elfroot, the quest pops up again that it's finished and ready to turn in.

 

I double checked with the requisition table and the Quartermaster, as well as the Revered Mother, but nothing.

Not with this particular quest, but this;
