I have both Focus perks, so all three focus bars.

Only one is filling up. When full, the other two are not filling during fights.

A known problem? Is there a fix?

I play the PC version.

I've heard about this bug.  I'm not 100% sure of the fix, but I think I've heard that saving and reloading fixes this.

I have found that if a character dies and is revived, they will only be able to fill one focus bar until you load a save or travel to another zone.

So I've tried reloading. I tried emptying my focus, traveling to Skyhold, saving, shutting out of everything and then restarting the game. I'm stuck with only 1 functional focus bar.

 

I did notice it happening after my main character was revived twice in a fight. So... could be that you should not use revive if you have upgraded focus. Awesome feature. Soooooo much damned time I spent on this play through and now it's broken. Wonderful.

Just check your inquisition perks if you actually did unlock it. No harm in checking.

Reloading the whole game, as in on my computer, or exiting and reloading that particular play through? My game took many hours to download.

Just loading a previous save from within the game fixed it for me. No need to exit.

I had been too far into the game by the time I noticed it. I was new to using focus and I wasn't paying attention. That's a horrible bug.
