How do conversations/romances work?

I've been getting a cutscene with Cassandra pretty much constantly, but it seems like I'm blazing through conversations with her really quickly. I've always had high approval but its been a constant string of cutscenes to the point I'm actually spacing them out and not talking to her for a few missions. Compared with Blackwall who I haven't had a proper cutscene with since Haven, Solace hasn't said a peep but "Tell me about Corpheus" since we arrived.

 

I'm a little confused on how triggering cutscenes work. Is it based purely on approval or triggering certain points in the story (like in DA:O and Mass Effect)? Heck, I've had a constant string of decent approval with Blackwall and still nada. Is it just a case that Cassandra got more content than the others?
 

On that note, I hit the scene where Cassandra confronts the Inquisitor about the flirting. Since I noticed she and the Inquisitor walk through a dozen incomplete wooden scaffolds I assumed this conversation wasn't meant to take place quite yet in the story, since I'm keen to not blaze through these companions in one go and never speak to them again. I've currently got the option of the Grey Warden fortress siege and the Orlesean Masquerade missions. Does waiting until after I finishing those have any effect or should I continue with it before I do them? Any hints on how to get conversations out of anybody else but Cassandra?
