I did several solos to track the number of kills on Routine and tracked the weapons kills possible per session. After a hung up operation with the Red Commander my challenge for "Greatsword" was thrown off when I entered in the the next game session. After that game session my Greatsword challenged was changed again.
The operation hanging up after defeating the Red Commander is a known issue (as least it should be). Here's the kit I was using:
Katari lvl 13
Sulevin Blade (superb lightning rune)
Deathbringer Hide (bas) w/ BM ArmorArms and BM CoatLegs
Superb amulet of Dex, Superb Belt of Health, Enhance Ring of Barrier Penetration, Enhanced Ring of Life-Drain
Abilities: Whirlwind, Mighty Blow, Charging Bull, War Horn/Passives down the main tree branch plus Coup de Grace.
Location: Elvin Ruins, Routine, Private
After the hung up game, I went into a Public Lobby (I meant to start a new solo), I exited the Lobby and started a new Solo (with Demon Commander). I got 203 more weapons kills then what I should've started the game with. When I completed the Demon Commander game (I died) session my weapon kills were dropped back to 2106.
In short, not only was I cheated out of +80 kills from my Demon Commander play but I was also dropped back another 20 kills from the Red Commander play. I was also timing each test as well. The RC took 23 mins and the DC took 12 mins. So that 35 mins of lost play. I wasn't tracking XP at the time so who knows what happened there.

This isn't a one-off issue because I seen the exact same behavior the Poison Challenge after a failed game. I was going to write it up but was unable to duplicate the steps to see why the numbers kept rolling back game-after-game. My poison kills kept resetting under 100 after 3 games straight then the challenge finally decided to "repair" itself. Mind you, I wasn't rewarded for the kills I earned. The Poison challenge finally "decided" to stop resetting, so I was probably 90 kills ahead of where it should have been.
Multiply this one tracked instance by the 30 (VERY conservative figure) or so odd failed games where XP or Challenges points weren't being properly rewarded. Whatever fix that was supposed to give players their credits back isn't working or made things worse. *sigh* Fix please.
